A sleep apnea machine works by pushing a steady stream of pressurized air into your airway while you sleep, preventing the soft tissue in your throat from collapsing and blocking your breathing. The air acts like an invisible splint, holding your airway open so oxygen flows freely all night. Most machines deliver pressure in a range of 4 to 20 centimeters of water pressure (cm H2O), with the average person needing around 8 to 10 cm H2O to keep breathing interruptions at bay.
The Core Mechanism: A Pneumatic Splint
During sleep apnea, the muscles in your throat relax enough that the surrounding tissue sags inward, partially or fully blocking your airway. Each time this happens, your brain briefly wakes you to restore muscle tone and reopen the passage. This cycle can repeat dozens of times per hour. Severity is measured by counting these events: fewer than 15 per hour is considered mild, 15 to 30 is moderate, and 30 or more is severe.
A CPAP (continuous positive airway pressure) machine solves this by creating a column of pressurized air that flows through a mask into your nose, mouth, or both. That pressurized air pushes outward against the walls of your throat, counteracting the tendency of relaxed tissue to collapse. Your airway stays open without your brain needing to jolt you awake, so you sleep through the night continuously.
What’s Inside the Machine
The machine itself is roughly the size of a small lunchbox and sits on your nightstand. Inside, a small motor draws in room air and pressurizes it to the prescribed level. A filter on the air intake traps dust, pollen, and other particles before they enter the system. From the motor, air travels through a flexible hose to a mask on your face.
Most modern machines include a built-in heated humidifier. This adds moisture to the pressurized air before it reaches you, which helps prevent the dry mouth, nasal congestion, and throat irritation that can come from breathing machine-dried air for eight hours straight. Some machines also use a heated hose that keeps moisture from condensing inside the tubing on cooler nights.
Three Types of PAP Machines
All positive airway pressure machines work on the same principle, but they deliver that pressure differently.
- CPAP (continuous): Delivers one fixed pressure level all night, the same whether you’re breathing in or out. Your pressure is determined during a sleep study and programmed into the machine before you take it home.
- APAP (auto-adjusting): Uses built-in software to monitor your breathing patterns in real time. Your doctor sets an upper and lower pressure limit (often something like 5 to 20 cm H2O), and the machine adjusts within that range on its own. If it detects signs of airway narrowing, it bumps the pressure up. During lighter sleep stages when your airway is more stable, it can drop the pressure back down.
- BiPAP (bilevel): Delivers two different pressure levels, a higher one when you inhale and a lower one when you exhale. This makes breathing out feel more natural, since you’re not pushing against full pressure on every breath. Some BiPAP machines can also regulate your breathing rate and the volume of air in each breath, which is useful for people with certain lung or neuromuscular conditions.
How Auto-Adjusting Machines Detect Problems
APAP and auto-BiPAP machines don’t just blow air at a set rate. They continuously analyze the shape of each breath you take. When your airway starts to narrow, the airflow pattern changes in a characteristic way: the peak of each inhale flattens out instead of forming a smooth, rounded curve. The machine’s algorithm recognizes this flattened pattern as a sign of partial obstruction and increases pressure before a full blockage occurs.
These algorithms also detect vibrations caused by snoring and sudden drops in airflow that signal a complete pause in breathing. The response is fast enough to prevent most events from waking you. That said, no auto-adjusting machine catches every subtle change perfectly. The technology has improved significantly, but there’s still some variation in sensitivity between brands and models.
Comfort Features That Help You Adjust
One of the biggest hurdles with PAP therapy is that breathing against pressurized air can feel unnatural, especially at first. Modern machines include several features designed to make the experience easier.
The ramp setting gradually increases pressure over the first 30 to 45 minutes after you turn the machine on. You fall asleep at a low, barely noticeable pressure, and the machine slowly climbs to your therapeutic level by the time you’re in deeper sleep. This avoids the sensation of a blast of air hitting you the moment you’re trying to drift off.
Expiratory pressure relief (often labeled EPR or a similar abbreviation depending on the manufacturer) briefly reduces pressure each time you breathe out. You can usually set this on a scale of 0 to 3, where 0 is off and 3 provides the most relief. This is especially helpful if your prescribed pressure is high, since exhaling against strong incoming air can feel like breathing into a headwind. The pressure never drops below 4 cm H2O, because that minimum is needed to keep the airway supported and flush out exhaled carbon dioxide from the mask.
Choosing a Mask
The mask is your main point of contact with the machine, and getting the right one matters as much as the machine itself. There are three main styles.
Nasal pillow masks are the smallest option. Two soft cushions sit just inside or against your nostrils. They leave most of your face uncovered, which works well if you feel claustrophobic in larger masks, want to wear glasses, or have facial hair that prevents other masks from sealing properly.
Nasal masks cover your entire nose but not your mouth. They offer a stable seal and work for most people who breathe through their nose during sleep.
Full-face masks cover both your nose and mouth. These are the go-to option if you tend to breathe through your mouth at night or if nasal congestion makes nose-only breathing unreliable. They’re bulkier, but they ensure the pressurized air reaches your airway regardless of how you breathe.
What Consistent Use Does for Your Body
When your airway stays open all night, the downstream effects go beyond simply feeling less tired. Each breathing pause during untreated sleep apnea triggers a small stress response: your blood oxygen drops, your heart rate spikes, and your blood pressure surges. Multiply that by dozens of events per hour, every night, and the cardiovascular strain adds up.
Consistent PAP therapy eliminates or dramatically reduces these events. Daytime sleepiness improves, often noticeably within the first week. Quality of life scores improve. Blood pressure tends to decrease, particularly in people whose high blood pressure has been resistant to medication. The machine tracks your nightly data, including how many breathing events still occur, how many hours you wore the mask, and whether there were mask leaks, so both you and your doctor can monitor how well the therapy is working over time.
