How Does an Ectopic Pregnancy Resolve Itself?

An ectopic pregnancy can sometimes resolve on its own, without medication or surgery. Roughly 30 to 50 percent of ectopic pregnancies end through spontaneous resolution, where the pregnancy stops developing and the body reabsorbs the tissue naturally. But this only happens safely under close medical monitoring, and it depends heavily on specific hormone levels at the time of diagnosis.

What Happens During Natural Resolution

In a normal pregnancy, a fertilized egg implants in the uterus. In an ectopic pregnancy, it implants somewhere else, most often in a fallopian tube. The tube cannot support a growing pregnancy, and the embryo is not viable regardless of how the ectopic is managed.

When an ectopic pregnancy resolves on its own, the embryonic tissue stops growing and begins to break down. Your body gradually reabsorbs it, and the pregnancy hormone hCG (the same hormone detected by pregnancy tests) steadily drops back to undetectable levels. This process is sometimes called expectant management, meaning the medical team watches and waits rather than intervening with drugs or surgery. You won’t pass tissue the way you would with a uterine miscarriage. The resolution happens internally, often with mild cramping and light vaginal bleeding that tapers off as hormone levels fall.

Who Qualifies for Expectant Management

Not every ectopic pregnancy is a candidate for watchful waiting. Doctors use hCG levels to predict whether natural resolution is likely to succeed. In a study of 210 patients with tubal ectopic pregnancies, those with hCG below 650 IU/L at the time of discharge and a decline of 50 percent or more during their initial hospital stay had a 97 percent success rate with expectant management. That’s a very high likelihood of the pregnancy resolving without any further treatment.

The picture changes quickly at higher hormone levels. Patients with hCG at or above 1,000 IU/L had only a 50 percent chance of successful resolution, regardless of whether levels were already declining. For patients falling between those thresholds, success rates sat around 76 percent. Higher hCG generally means more active pregnancy tissue, which increases both the chance that the pregnancy will keep growing and the risk of complications like tubal rupture.

Beyond hormone levels, your medical team will also consider the size of the ectopic mass on ultrasound, whether there’s a visible heartbeat (which rules out expectant management), and whether you’re experiencing significant pain or signs of internal bleeding. You also need to be able to return for frequent follow-up appointments and get to an emergency room quickly if symptoms worsen.

What Monitoring Looks Like

If you and your doctor decide on expectant management, you’ll have blood drawn frequently to track your hCG levels. The typical schedule starts with a repeat blood test 48 hours after diagnosis. If the level is falling, another test follows 48 hours after that, then another 72 hours later. Once the decline is clearly established, monitoring shifts to weekly blood draws until hCG drops below 20 mIU/ml, which is essentially an undetectable level.

A repeat ultrasound usually isn’t necessary unless your symptoms change, your pain increases, or your hCG levels plateau or start rising. Rising levels are a red flag that the pregnancy is still active, and your team will typically move to medical treatment (a medication that stops the cells from growing) or surgery at that point. The entire process from diagnosis to undetectable hCG can take several weeks, sometimes longer. During that time, you’ll need to watch for warning signs of rupture: sudden sharp pain on one side of your lower abdomen, shoulder tip pain (caused by internal bleeding irritating the diaphragm), dizziness, or fainting.

When Natural Resolution Fails

Even with declining hCG, expectant management doesn’t always work. Some ectopic pregnancies stabilize at a low hormone level without fully resolving, or they begin growing again after an initial drop. In those cases, medical or surgical treatment becomes necessary. Tubal rupture is the most serious risk. A ruptured fallopian tube causes internal bleeding that can become life-threatening quickly, requiring emergency surgery. This is why expectant management is only offered when the initial numbers strongly favor spontaneous resolution and when the patient has reliable access to emergency care.

Fertility After Natural Resolution

One common concern is whether an ectopic pregnancy, however it’s managed, will affect your ability to get pregnant in the future. The evidence is reassuring. A study comparing tubal patency (whether the fallopian tubes remain open and functional) across different management approaches found that 78 percent of patients who resolved naturally had a patent tube on the affected side. That’s comparable to the 84 percent rate seen with medication. On the unaffected side, tubal patency was 92 percent after expectant management, 97 percent after medication, and 83 percent after surgical removal of the tube. None of these differences were statistically significant.

In practical terms, this means natural resolution does not appear to damage your fallopian tubes any more than other treatment options. Having one ectopic pregnancy does increase your overall risk of another one in the future, regardless of how it was treated, but most people who have had an ectopic pregnancy go on to have successful pregnancies afterward.
