It reduces the risk of cardiovascular death and hospitalization for heart failure across the full spectrum of heart failure types, and it’s now considered one of four core medication classes recommended for treating heart failure with reduced pumping ability.
How Jardiance Removes Fluid Differently Than Other Diuretics
The most immediate way Jardiance helps is by pulling excess fluid out of the body. It works in the kidneys by blocking a protein called SGLT2, which normally reabsorbs sugar back into the bloodstream. When that protein is blocked, sugar passes into the urine, and water follows it through osmotic pressure. The result is a gentle, sustained diuresis that reduces the fluid overload responsible for swelling, shortness of breath, and strain on the heart.
What makes this mechanism particularly useful is where the fluid comes from. Traditional diuretics like furosemide pull fluid from the blood vessels and the spaces between cells somewhat indiscriminately. Jardiance preferentially draws fluid from the interstitial space, the tissue between organs and blood vessels where fluid accumulates during heart failure. This distinction matters because removing too much fluid from blood vessels can drop blood pressure and stress the kidneys, while clearing interstitial fluid relieves congestion without those trade-offs.
A comparative study of heart failure patients found that adding Jardiance produced greater weight loss (5.6 kg) than doubling the furosemide dose (3.6 kg) over one month. More importantly, Jardiance did this without significantly changing sodium or potassium levels, while the higher furosemide dose caused measurable drops in both electrolytes. That electrolyte stability is a real advantage, since low sodium and potassium are common complications of aggressive diuretic therapy and can trigger dangerous heart rhythm problems.
How It Changes the Heart’s Energy Supply
Beyond fluid removal, Jardiance appears to reshape how the heart muscle fuels itself. A failing heart is an energy-starved heart. It struggles to produce enough cellular energy to contract effectively, and it becomes overly dependent on glucose as a fuel source, which is less efficient than alternatives.
Jardiance shifts the heart’s metabolism in several ways. It increases levels of a compound called beta-hydroxybutyrate, a ketone body that the heart can burn for energy. Per unit of weight, ketone bodies generate more cellular energy than glucose does. In animal studies, this ketone boost improved cardiac performance and reduced harmful structural changes in the heart. The drug also enhances fatty acid uptake into heart cells, giving the muscle another efficient fuel source. Studies measuring the heart’s energy reserves have found that Jardiance improves the ratio of available energy compounds in heart tissue, likely driven by its effects on blood pressure and weight loss.
Protecting Heart Cells From Calcium Overload
In heart failure, calcium handling inside heart muscle cells goes wrong. Too much calcium builds up in the cell, which weakens contractions and can damage the muscle over time. Jardiance counteracts this by blocking a sodium-hydrogen exchanger on heart cells, which indirectly lowers calcium accumulation inside the cell. It also inhibits an enzyme that controls calcium release from internal storage compartments, slowing the leak of calcium and allowing heart muscle to contract more forcefully. The FDA approved Jardiance for heart failure based on two landmark trials. The first, EMPEROR-Reduced, enrolled adults with chronic heart failure whose hearts pumped out 40% or less of their blood volume with each beat (a normal heart ejects about 55% to 70%). In this population, Jardiance was given on top of standard heart failure therapy.
The FDA indication that came from this trial is broad: Jardiance is approved to reduce the risk of cardiovascular death and hospitalization for heart failure in adults with heart failure. It can be used in patients with kidney function as low as an eGFR of 20, a threshold that excludes very few heart failure patients.
Trial Results in Preserved Ejection Fraction
The second trial, EMPEROR-Preserved, tested Jardiance in patients whose hearts pumped out more than 40% of their blood per beat. This type of heart failure, where the heart squeezes adequately but stiffens and doesn’t fill properly, has historically been much harder to treat. Few medications had shown clear benefits for it before SGLT2 inhibitors came along.
Jardiance reduced the combined risk of cardiovascular death or hospitalization for heart failure by 21% compared to placebo. That benefit was driven primarily by a 29% reduction in heart failure hospitalizations. When researchers looked at total hospitalizations for heart failure (counting repeat admissions, not just first events), Jardiance cut that number by 27%. The effect on cardiovascular death alone was not statistically significant, but the reduction in hospitalizations was substantial and consistent across patient subgroups.
Where Jardiance Fits in Treatment Guidelines
The 2022 guidelines from the American Heart Association, American College of Cardiology, and Heart Failure Society of America place SGLT2 inhibitors like Jardiance among the four foundational medication classes for heart failure with reduced ejection fraction. This is the highest level of recommendation, meaning the evidence strongly supports using it in virtually all eligible patients alongside the other three core therapies.
For patients with mildly reduced or preserved ejection fraction, the guidelines give SGLT2 inhibitors a slightly lower but still favorable recommendation, reflecting the strong trial data in these populations. This makes Jardiance one of the very few medications recommended across the entire range of heart failure types.
What Taking Jardiance Looks Like
The standard dose for heart failure is 10 mg once daily, taken in the morning with or without food. If tolerated, the dose can be increased to 25 mg. There’s no complex titration schedule like some other heart failure medications require, which simplifies the process for both patients and clinicians.
Because Jardiance causes sugar to spill into the urine, you’ll urinate more frequently, especially early on. This same mechanism means genital yeast infections are more common, since the sugar-rich urine creates a favorable environment for yeast. Staying hydrated and maintaining good hygiene reduces this risk. Blood pressure tends to drop modestly, which is generally beneficial in heart failure but worth monitoring if you’re already on multiple blood pressure-lowering medications.
One important practical point: Jardiance works for heart failure regardless of whether you have diabetes. The heart-related benefits operate through the mechanisms described above, not through blood sugar control. If you don’t have diabetes, the drug won’t cause dangerous drops in blood sugar because your body’s other regulatory systems keep glucose levels stable.
