How Does Narcan Work in the Body: Reversing Overdose

Narcan (naloxone) works by knocking opioids off the receptors in your brain that control breathing, consciousness, and pain. It competes for the same binding sites that opioids like fentanyl, heroin, and oxycodone attach to, and because naloxone binds to those sites without activating them, it effectively blocks the opioid’s effects within minutes. The result: a person whose breathing has slowed dangerously or stopped can start breathing again.

How Naloxone Displaces Opioids at the Receptor

Your brain has proteins called opioid receptors, and the most important one for overdose is the mu receptor. When an opioid molecule locks onto a mu receptor, it triggers a cascade of effects: pain relief, sedation, euphoria, and, critically, slower breathing. In an overdose, so many receptors are activated that the brain’s breathing signals can shut down entirely.

Naloxone is what pharmacologists call a competitive antagonist. It races to those same mu receptors and latches on, physically displacing the opioid molecules already sitting there. But unlike an opioid, naloxone doesn’t flip the receptor “on.” It just occupies the space, blocking opioids from reattaching. Think of it like changing the locks: the opioid still exists in the bloodstream, but it can no longer get through the door. Naloxone also binds to other opioid receptor types (kappa and sigma), but its strongest pull is toward the mu receptor, which is the one most responsible for the life-threatening breathing suppression in an overdose.

How Quickly It Works and How Long It Lasts

Speed depends on how naloxone enters the body. Given through a vein, it takes effect in one to two minutes. An injection into the muscle works on a similar timeline. The nasal spray version, which is the form most people encounter, takes a bit longer because the drug has to absorb through the lining of the nose. Expect three to seven minutes for the nasal spray to kick in.

Here’s the crucial part: naloxone wears off faster than most opioids do. Its half-life is roughly 30 to 45 minutes, and its total duration of action runs about 90 minutes to three hours depending on the dose. Many opioids, especially long-acting ones or large doses of fentanyl, last longer than that. This means an overdose can return after naloxone fades. The person may stop breathing again once the naloxone clears and the opioid molecules reclaim those receptors.

Why Fentanyl Can Require Multiple Doses

Not all opioids grip the mu receptor with the same strength. Naloxone has a binding affinity (measured as a Ki value) of about 18 nanomolar. Fentanyl’s binding affinity is around 1.6 nanomolar, meaning it grips roughly 10 times tighter. Carfentanil, an even more potent synthetic opioid, binds at 0.024 nanomolar, hundreds of times tighter than naloxone.

In practical terms, this means a standard dose of naloxone can be outcompeted when the opioid involved is extremely potent or present in large amounts. One spray may not displace enough fentanyl molecules to restore breathing. That’s why the FDA-approved instructions for the nasal spray recommend giving a second dose after two to three minutes if there’s no response, and continuing to give additional doses every two to three minutes (using a new spray each time) until help arrives. Each dose adds more naloxone molecules to the competition, gradually tipping the balance.

Withdrawal Symptoms After Naloxone

Because naloxone strips opioids off receptors so abruptly, it can trigger what’s known as precipitated withdrawal. This is different from the gradual withdrawal someone might experience when they stop using opioids on their own. Precipitated withdrawal hits fast, often within minutes of the naloxone dose, and it can be intense.

Common symptoms include irritability, anxiety, yawning, abdominal cramping, nausea, vomiting, flushing, sweating, and a racing heart. Some people become agitated or combative when revived. These symptoms are deeply uncomfortable but not life-threatening on their own. They happen because the brain’s opioid system, which had been flooded, is suddenly running on empty. The discomfort typically tracks with the naloxone’s duration, peaking within the first hour and gradually easing as the drug is metabolized.

What Naloxone Cannot Reverse

Naloxone only works on opioid receptors. It has no effect on other substances. If an overdose involves alcohol, benzodiazepines, stimulants, or other non-opioid drugs alongside opioids, naloxone will reverse the opioid component but leave the other effects untouched.

This distinction matters more now than ever because of xylazine, a veterinary sedative increasingly mixed into the fentanyl supply. The CDC has stated plainly that naloxone will not reverse the effects of xylazine. However, because xylazine is almost always found alongside fentanyl, naloxone should still be given to address the opioid portion of the overdose. The xylazine effects on breathing may continue even after naloxone restores some function, which is why rescue breathing and calling 911 remain essential. First responders have noted that rescue breaths are especially important in suspected xylazine cases because the drug independently suppresses breathing.

How the Body Processes Naloxone

Once naloxone has done its job at the receptors, the liver breaks it down. The drug is metabolized relatively quickly, which accounts for its short window of action. This is why emergency medical care is still necessary even after a successful reversal. The opioid may still be circulating in the body long after the naloxone has been cleared, and without medical monitoring, the overdose can resume.

Naloxone itself has no abuse potential and is not a controlled substance. It produces no high, no sedation, and no psychoactive effects in a person who doesn’t have opioids in their system. If given to someone who isn’t experiencing an opioid overdose, it simply does nothing. The FDA approved the first over-the-counter naloxone nasal spray in March 2023, and a second OTC product has since been approved, making it available without a prescription at pharmacies nationwide.
