Spironolactone works for hair loss by blocking androgens, the hormones responsible for shrinking hair follicles in a pattern called androgenetic alopecia. It was originally developed as a blood pressure medication, but its ability to interfere with androgen activity made it a widely used off-label treatment for hair thinning, particularly in women. About 81% of women taking it show improvement in hair growth within a year.
How It Targets Hair Loss at the Follicle
Hair thinning in androgenetic alopecia happens because androgens (especially a potent form of testosterone called DHT) bind to receptors on hair follicles and cause them to gradually miniaturize. Each growth cycle produces a thinner, shorter strand until the follicle eventually stops producing visible hair altogether.
Spironolactone fights this process on two fronts. First, it acts as a competitive blocker at androgen receptors in the scalp, physically preventing DHT and testosterone from latching onto the follicle and triggering miniaturization. Think of it as occupying the parking spot that DHT needs. Second, it reduces testosterone production in the adrenal glands by interfering with the enzymes that build the hormone. This lowers the overall amount of androgen circulating in the body, which means less DHT reaching the scalp in the first place.
Because it works by reducing androgen activity rather than stimulating hair growth directly, spironolactone is better at stopping further loss and allowing weakened follicles to recover than at regrowing hair on areas that have been bare for years. That’s why it’s often paired with other treatments that actively stimulate growth.
Who It’s Prescribed For
Spironolactone is primarily used for women with pattern hair loss. It is not typically prescribed to men because blocking androgens at the doses needed for hair results can cause breast tissue growth and other feminizing effects in male patients. For men, other androgen-related treatments with a narrower mechanism are generally preferred.
In women, it’s especially useful when hair thinning follows the classic female pattern: gradual widening of the part line and diffuse thinning across the top of the scalp rather than a receding hairline. Dermatologists sometimes check androgen levels before prescribing, but elevated lab values aren’t always required. Many women with normal hormone levels still respond well because the drug reduces androgen activity at the follicle itself.
Typical Dosing
The average dose used for hair loss is 100 mg daily, though prescriptions range from 25 mg to 200 mg depending on the individual. Many dermatologists start at 25 or 50 mg and increase gradually over several weeks to minimize side effects. This slow ramp-up gives your body time to adjust to the drug’s diuretic and hormonal effects.
A topical formulation (1% spironolactone gel) also exists and has shown clinical improvement in 80% of patients in a 12-month trial, though it’s far less commonly available than the oral form.
When to Expect Results
Spironolactone is not a fast fix. Hair follicles cycle slowly, so visible changes take time. Most clinical assessments don’t even begin until at least six months of continuous use, and the majority of patients see their best results after a full year or longer. In one study of 45 women tracked over time, 64% achieved their peak improvement at the one-year mark or beyond.
During the first few months, you’re unlikely to notice much change in the mirror. Some people experience a brief period of increased shedding early on as follicles reset their growth cycles, which can feel discouraging but is generally temporary. The key is consistency: stopping and restarting the medication resets the timeline.
Combining It With Minoxidil
Because spironolactone primarily prevents further damage while minoxidil actively stimulates new growth, the two are frequently prescribed together. Spironolactone handles the hormonal side of the problem; minoxidil increases blood flow to the follicle and extends the active growth phase of each hair.
An interesting benefit of starting both at the same time: spironolactone’s anti-androgen effects can counteract the unwanted facial and body hair growth (hypertrichosis) that minoxidil sometimes causes. One study found that simultaneous initiation of both medications reduced the risk of hypertrichosis by about 65% compared to starting minoxidil alone. The diuretic properties of spironolactone may also help offset the mild fluid retention some people experience with oral minoxidil.
Common Side Effects
Spironolactone was designed as a potassium-sparing diuretic, so increased urination is one of the most noticeable early effects. Most people adjust within a few weeks. Beyond that, the most commonly reported side effects relate to its hormonal activity:
- Menstrual irregularities: Lighter, heavier, or less predictable periods are common, especially in the first few months.
- Breast tenderness or swelling: The anti-androgen effect can cause breast sensitivity in some women.
- Lightheadedness: Because the drug lowers blood pressure slightly, some people feel dizzy when standing up quickly, particularly at higher doses.
- Fatigue: Mild tiredness can occur as the body adjusts to fluid and electrolyte shifts.
These effects are generally dose-dependent. Starting low and titrating up makes them more manageable, and many subside within the first month or two.
Potassium and Blood Work
Because spironolactone causes the kidneys to retain potassium, there has historically been concern about dangerously high potassium levels (hyperkalemia). In practice, this risk is very low in young, healthy women with normal kidney function. Current dermatology guidelines no longer recommend routine potassium monitoring for young, healthy women taking spironolactone for skin or hair conditions.
For women over 45, or anyone with kidney problems, heart conditions, or who takes other medications that raise potassium (like certain blood pressure drugs), periodic blood work is still standard. Your prescriber will determine the right monitoring schedule based on your health profile.
Pregnancy and Fertility
Spironolactone can interfere with fetal development because of its anti-androgen properties, so reliable contraception is important while taking it. If you’re planning a pregnancy, the drug is typically stopped in advance. Fertility and menstrual cycles generally return to normal within about two months of discontinuation.
The drug can also cause irregular periods while you’re on it, which can make tracking ovulation more difficult. This effect reverses after stopping treatment.
What Happens if You Stop
Spironolactone does not permanently change your hormone levels or follicle biology. It works only while you’re taking it. Once you stop, androgen activity returns to its previous level, and follicle miniaturization can resume. Most dermatologists consider it a long-term maintenance medication rather than a course of treatment with a defined endpoint. Women who respond well typically continue taking it indefinitely, or until their priorities change (such as planning a pregnancy).
