Most ulcers don’t reach that point, but the ones that do create a surgical emergency where every hour counts. Understanding the timeline, the warning signs, and what pushes an ulcer from painful to fatal can help you act fast enough to change the outcome.

How a Stomach Ulcer Becomes Fatal

A stomach ulcer on its own is a painful open sore in the lining of the stomach or upper intestine. It can bleed, cause vomiting, and make a dog miserable, but it won’t necessarily kill them. The danger comes from two paths: severe internal bleeding or perforation.

With perforation, the ulcer erodes completely through the stomach wall, creating a hole. Stomach acid, partially digested food, and bacteria spill into the abdominal cavity. This triggers a condition called septic peritonitis, which is the leading killer. The body responds with massive inflammation: blood vessels dilate, fluid shifts out of the bloodstream and into the abdomen, blood pressure drops, and organs begin to starve for oxygen. The heart, kidneys, and lungs can start failing in a cascade. Perforating ulcers account for a significant portion of septic peritonitis cases in dogs, with gastrointestinal causes making up 53% to 75% of all cases.

In one documented case at Cornell University’s veterinary hospital, a dog went from eating treats normally to trembling, vomiting, and refusing food within about 12 hours. By the time it arrived at the hospital, it was febrile, had an elevated heart rate, and was reluctant to walk. Despite surgery, the repair broke down within days and euthanasia was chosen due to the severity of the infection and the dog’s critical condition.

The Timeline From Perforation to Death

Once a stomach ulcer perforates, the clock moves fast. Within hours, bacteria begin multiplying in the abdomen. The body’s immune response releases a flood of inflammatory signals that damage blood vessel walls, cause widespread clotting problems, and redirect blood flow away from vital organs. This can progress to full septic shock within 12 to 24 hours.

Without surgery, most dogs with a perforated ulcer will not survive more than a day or two. Even with emergency surgery, the survival rate for dogs with septic peritonitis from gastrointestinal causes is only about 53%. That means roughly half of dogs who make it to the operating table don’t make it home. Dogs that also develop kidney failure or clotting disorders during the process fare significantly worse.

Slow internal bleeding from a non-perforated ulcer follows a longer but still dangerous timeline. A dog can lose blood gradually over days to weeks, becoming progressively weaker and more anemic before collapsing. This is harder to detect because most dogs with upper GI bleeding don’t show obvious changes in their stool color. The classic sign, pitch-black tarry stool, only appears when the bleeding is substantial. A simple test your vet can do involves pressing fresh stool onto white paper to see if a reddish color diffuses out.

Warning Signs That an Ulcer Is Critical

The early signs of a stomach ulcer in dogs are easy to dismiss: occasional vomiting, decreased appetite, mild lethargy. The signs that indicate a crisis are different and more dramatic.

  • Vomiting blood: This can look like bright red blood or dark, coffee-ground-like material.
  • Hunched posture or rigid abdomen: A dog with abdominal pain from peritonitis will often stand with an arched back, head low, and resist being touched on the belly.
  • Pale or white gums: This signals blood loss or shock. Healthy gums should be pink and return to pink within two seconds after you press them.
  • Rapid heart rate with weakness: The body compensates for blood loss by speeding up the heart. Combined with reluctance to move, this is a red flag.
  • Sudden collapse: If internal bleeding or sepsis has been progressing silently, the first obvious sign can be the dog going down.

Any combination of vomiting blood, black stool, abdominal pain, and pale gums warrants an emergency vet visit, not a “wait and see” approach.

What Causes Ulcers to Form Quickly

Some ulcers develop over weeks. Others can form in days, depending on the cause.

NSAIDs are the most common culprit. Dogs are far more sensitive to these drugs than humans. Ibuprofen (Advil, Motrin) can cause gastrointestinal damage at doses as low as 25 mg per kilogram of body weight. For a 10-kilogram dog (about 22 pounds), that’s just one or two standard human tablets. Even veterinary NSAIDs like carprofen can cause ulceration at higher-than-prescribed doses or when combined with steroids. The damage happens through two routes: direct irritation of the stomach lining and suppression of the protective mucus layer that normally shields it from acid.

Mast cell tumors are another serious cause. These common skin cancers in dogs release histamine, which drives the stomach to overproduce acid. The excess acid erodes the stomach lining, sometimes causing severe ulceration before the tumor itself is even diagnosed. Dogs with mast cell tumors can develop GI ulcers as a secondary complication that becomes life-threatening on its own.

Other causes include liver or kidney disease, severe stress from illness or surgery, and foreign objects that damage the stomach wall. Corticosteroids, especially when combined with NSAIDs, dramatically increase ulcer risk.

How Vets Diagnose and Treat Severe Ulcers

Diagnosing a perforated ulcer is not always straightforward. No single test confirms it. Vets typically use a combination of abdominal ultrasound, X-rays, and sampling fluid from the abdomen. On ultrasound, the key finding is bright, abnormal-looking fat surrounding the stomach or intestines, which signals inflammation and possible leakage. Free air in the abdomen on X-ray also suggests perforation, though it’s not always present.

For ulcers that haven’t perforated, treatment focuses on stopping the acid assault and protecting the damaged tissue. Proton pump inhibitors, the same class of drug as human heartburn medications, are the most effective option for reducing stomach acid. A protective coating agent that binds directly to the ulcer crater is often given alongside. If NSAID use caused the ulcer, stopping the drug immediately is critical, and a synthetic protective compound may be added to help the lining recover.

For perforated ulcers, surgery is the only option. The surgeon cleans the abdomen, repairs or removes the damaged section of stomach, and the dog typically stays in intensive care for days afterward. Even after successful surgery, the repair can break down. In the Cornell case, the surgical site failed within two days of discharge, requiring a second emergency surgery that ultimately could not save the dog.

What Affects a Dog’s Chances of Survival

The single biggest factor is how quickly treatment begins. A dog that reaches surgery within hours of perforation has a meaningfully better chance than one that has been septic for a full day. The overall survival rate for dogs with septic peritonitis who undergo surgery is roughly 52%, but that number hides a wide range. Dogs that are stable going into surgery, without kidney failure or clotting problems, do considerably better than those already in organ failure.

Age, overall health, and the underlying cause also matter. A young dog with a single NSAID-induced ulcer that’s caught early has a very different prognosis than an older dog with a mast cell tumor and multiple ulcers. Dogs on long-term steroid or NSAID therapy are at higher risk for ulcers that go undetected until they’re advanced, because the drugs can mask pain and inflammation that would otherwise prompt an earlier vet visit.
