How Long After a Wasp Sting Can an Allergic Reaction Occur?

Most allergic reactions to a wasp sting happen within minutes, but the timeline varies depending on the type of reaction. A life-threatening anaphylactic reaction typically begins within 5 to 30 minutes of the sting. Other allergic responses can take hours or, in rare cases, days to appear.

Immediate Allergic Reactions: Minutes

The most dangerous reaction, anaphylaxis, is also the fastest. It usually starts within minutes of being stung and can escalate quickly. Your immune system floods the body with chemicals that cause a sudden drop in blood pressure, airway constriction, and widespread hives. The hallmark signs include difficulty breathing, a swollen tongue or throat, a weak and rapid pulse, dizziness, and flushed or pale skin. Nausea, vomiting, and diarrhea can also occur.

Anaphylaxis can be fatal if untreated. If you carry an epinephrine autoinjector, use it immediately. Even if symptoms improve after the injection, you still need emergency care because symptoms can return without a second exposure to venom.

Large Local Reactions: Hours to Days

Not every allergic reaction is anaphylaxis. A large local reaction causes dramatic swelling and redness that extends well beyond the sting site, sometimes covering an entire limb. These reactions typically start 6 to 12 hours after the sting and continue to worsen over one to two days before gradually fading. The whole process can last 5 to 10 days.

Large local reactions look alarming, but they carry a relatively low risk of progressing to something more serious. People who have experienced a large local reaction have less than a 10% chance of a systemic reaction to a future sting, and less than a 5% chance of full anaphylaxis. Because the risk is low, allergy immunotherapy (venom shots) is not generally recommended for people whose worst reaction has been local swelling.

That said, these reactions can be genuinely uncomfortable. The swelling can be significant enough to limit movement in the affected area, and the peak discomfort at 24 to 48 hours can catch people off guard if they assumed the worst was over shortly after being stung.

Delayed Reactions: Days to Weeks

In rare cases, allergic symptoms don’t appear until well after the sting. Some people develop widespread hives 6 to 24 hours later. Others have reactions surface 3 days or even 1 to 2 weeks after the sting. A review from the American Academy of Allergy, Asthma & Immunology documented 10 patients with delayed reactions: four developed a serum sickness-type illness (joint pain, fever, and general malaise), five broke out in generalized hives, and two experienced throat swelling days after the sting.

One notable case involved a patient who developed hives and significant facial and hand swelling a full week after being stung, with symptoms that didn’t respond to antihistamines. These delayed reactions are driven by the same venom-specific immune response as immediate anaphylaxis, just on a slower timeline.

Because delayed reactions are uncommon and can mimic other conditions (a viral rash, for instance), they’re easy to miss. If you develop unexplained hives, joint pain, or swelling in the days following a wasp sting, the sting is worth mentioning to a doctor even if it seems too long ago to be relevant.

Biphasic Reactions: The Second Wave

Even after anaphylaxis is treated and symptoms resolve, there’s a chance they can return. This is called a biphasic reaction, and it happens in roughly 5% of anaphylaxis cases. The second wave can occur hours after the initial episode with no new sting required.

This is why anaphylaxis guidelines recommend keeping patients under clinical observation for at least 4 to 6 hours after their last dose of epinephrine, and sometimes overnight. The second wave can be just as severe as the first, which is the reason emergency departments won’t send you home the moment you feel better.

How to Tell a Normal Sting From an Allergic Reaction

Every wasp sting hurts. The venom itself causes localized pain, redness, and a small area of swelling even in people with no allergy at all. A normal reaction stays confined to the sting site, peaks within a few hours, and resolves within a day or two.

An allergic reaction differs in scope. Watch for these warning signs that the reaction has moved beyond normal:

  • Swelling that spreads far from the sting, such as a sting on the hand causing the entire forearm to swell
  • Hives or itching on parts of the body away from the sting site
  • Breathing difficulty, throat tightness, or a hoarse voice
  • Dizziness, feeling faint, or a rapid heartbeat
  • Nausea, vomiting, or stomach cramps

Symptoms that stay local, even if the swelling is impressive, are a very different situation from symptoms that show up in areas unrelated to the sting. Hives on your chest after a sting on your ankle, for example, means the reaction has gone systemic.

What Determines Your Risk

Your reaction to one sting doesn’t perfectly predict your reaction to the next. Someone who had a large local reaction has a less than 10% chance of a systemic reaction next time. But someone who has already had anaphylaxis to a wasp sting has a significantly higher chance of it happening again, which is why allergists often recommend venom immunotherapy for that group.

Age and overall health matter too. Adults tend to have more severe systemic reactions than children. People taking certain blood pressure medications may find that anaphylaxis is harder to reverse with epinephrine, making the reaction more dangerous even at the same severity level.
