You can then continue alternating back and forth every 3 to 4 hours if needed.
Why Alternating Works
Both Motrin and Tylenol reduce pain and fever by blocking the same family of enzymes your body uses to produce chemicals called prostaglandins, which drive inflammation, pain signaling, and fever. The key difference is where each drug acts. Acetaminophen works only in the brain, while ibuprofen works in the brain and throughout the rest of the body. Because they block those enzymes at different locations, they don’t compete with each other or double up on the same pathway. This is what makes it possible to use them together or in a staggered pattern.
A Practical Alternating Schedule
A typical approach looks like this: give one medication, then 3 hours later give the other, and continue rotating. So if you give Motrin at noon, you’d give Tylenol at 3 p.m., then Motrin again at 6 p.m., and so on. Some sources recommend waiting 4 to 6 hours between the two rather than 3, so the exact spacing can vary. The important thing is to track each dose carefully, including which drug you gave and when, so you don’t accidentally double up on either one.
This alternating schedule is meant for short-term use only, generally no more than 24 hours. If pain or fever persists beyond that, it’s worth checking in with a doctor rather than continuing to rotate medications on your own.
Daily Limits to Watch
Even when alternating, each medication still has its own maximum daily dose. For acetaminophen, the ceiling is 4,000 milligrams in 24 hours for adults. Going over that threshold puts serious stress on the liver. For ibuprofen, most over-the-counter labels cap adults at 1,200 milligrams per day (three standard 400 mg doses), though a doctor may approve more in certain situations.
When you’re switching back and forth between two different drugs, it’s easy to lose track. Writing down each dose with the time and drug name is the simplest way to avoid accidentally exceeding either limit. This is especially important if other people in the household are also administering medication, such as when parents are taking turns caring for a sick child.
Alternating for Children
Parents commonly alternate Motrin and Tylenol for a child’s fever, and many pediatricians do recommend this approach. However, the practice has some notable cautions. There is limited clinical evidence that alternating the two drugs reduces fever faster or more effectively than using either one alone. The bigger concern is dosing errors: when you’re juggling two medications with different dosing intervals, the risk of giving too much of one goes up.
If you do alternate for a child, keep it to 24 hours or less. Ibuprofen (Motrin) should not be given to babies under 6 months old, as it has not been established as safe for that age group and is not FDA-approved for infants that young. Acetaminophen (Tylenol) can be used in younger infants, but dosing should be based on the child’s weight, not age.
For children under 12, combination tablets containing both ibuprofen and acetaminophen are not appropriate. Dosing for younger children needs to be determined individually, ideally with guidance from a pediatrician, especially for infants.
Taking Both at the Same Time
You don’t strictly have to stagger them. Because the two drugs work at different sites in the body, taking Motrin and Tylenol at the same time is also considered safe for adults. A combination tablet containing both is available by prescription. The staggered approach is popular because it spreads out the pain or fever relief more evenly across the day, so you’re less likely to hit a window where neither drug is active. But if you need quick relief and both are due, taking them together won’t cause a harmful interaction, as long as you stay within each drug’s daily maximum.
