Most people with COVID-19 are contagious for about 5 to 10 days after symptoms start, with the highest risk of spreading the virus falling between days 2 and 5. After day 10, live virus is rarely detectable in people with mild or moderate illness. Your exact window depends on how sick you are, how your immune system functions, and whether you experience a rebound.
Peak Contagiousness: Days 2 Through 5
The virus replicates fastest in the first few days of illness. Studies on the Omicron variant found that 41.7% of samples containing live, culturable virus were collected between days 2 and 5 after diagnosis or symptom onset. This is the period when you’re most likely to infect someone else, even if your symptoms feel manageable.
Vaccinated individuals with mild or asymptomatic infections shed infectious virus for roughly 6 to 9 days after symptom onset. After day 9, no live virus was isolated in these studies. By day 10, viral levels drop sharply in most people, which is why the 10-day mark has long served as a practical cutoff for isolation in mild cases.
When You Can Spread It Without Symptoms
About 20% of people infected with COVID-19 never develop symptoms at all. They can still spread the virus to others without realizing they’re sick. If you test positive but feel fine, you’re still shedding virus during those early days, and you should treat yourself as contagious for at least 5 days after your positive test. Wearing a well-fitting mask around others during that window significantly reduces transmission risk.
Pre-symptomatic spread is also common. You can be contagious for 1 to 2 days before symptoms appear, which is one reason COVID spreads so efficiently. People are estimated to be most infectious roughly 4 days after initial infection, which often lines up with the day symptoms first show up or just before.
Current Isolation Guidelines
The general guidance for COVID-19 now mirrors the approach for other respiratory illnesses: stay home and avoid contact with others until you’ve been fever-free for at least 24 hours without using fever-reducing medication, and your other symptoms are improving. This replaces the older fixed-day isolation rules.
Once you feel better and return to normal activities, take extra precautions for the next five days. That means wearing a well-fitting mask, keeping distance when possible, choosing well-ventilated spaces, washing your hands frequently, and testing before gathering with others. You’re likely still shedding some virus during this window, even if the amount is declining.
Severe Illness and Weakened Immune Systems
If your illness is severe, requiring hospitalization or intensive care, you may remain contagious well beyond the standard window. The CDC recommends isolation for at least 10 days after symptom onset in severe cases, with the possibility of extending that to 20 days. Fever must be gone for at least 24 hours and other symptoms should be improving before isolation ends.
People with significantly weakened immune systems face the longest contagious periods. If you’re immunocompromised due to an organ transplant, cancer treatment, or a similar condition, you should isolate for at least 20 days. Some people in this group continue shedding live virus beyond that. Ending isolation typically involves serial testing: two consecutive negative tests taken at least 24 hours apart, ideally in consultation with a specialist. In rare cases where tests stay positive past 30 days, additional lab work can help determine whether you’re still producing infectious virus.
What Your Test Results Actually Mean
A positive rapid antigen test generally correlates well with the presence of live virus, making it a reasonable proxy for whether you’re still contagious. If you’re testing positive on a rapid test, assume you can still spread the virus. A negative rapid test is a good sign, though it’s not a guarantee, especially if you test very early in an infection.
PCR tests (also called NAATs) are a different story. These tests detect viral genetic material rather than live virus, and they can continue returning positive results for up to 90 days after infection. A positive PCR test weeks after recovery does not mean you’re still contagious. If you’re trying to determine whether it’s safe to be around others, rapid antigen tests are more useful than PCR for that purpose.
COVID Rebound and Returning Contagiousness
Some people feel better for a day or two, then experience a return of symptoms or a new positive test. This is called COVID rebound. It happens in roughly 1 in 5 people who take antiviral treatment, though a CDC review found no consistent link between treatment and rebound specifically. It can also happen in people who were never treated.
Rebound symptoms are generally mild, and no hospitalizations or deaths have been attributed to rebound episodes. The working theory is that once the antiviral clears the body after its 5-day course, residual virus can briefly ramp up again. If you experience rebound, treat yourself as contagious again. Restart your precautions: stay home until you’re fever-free for 24 hours with improving symptoms, then mask for another five days after that.
