The median survival time for cats diagnosed with feline leukemia virus (FeLV) is about 2.5 years, according to Cornell University’s College of Veterinary Medicine. But that number masks a wide range of outcomes. Some cats decline within weeks of diagnosis, while others live five years or longer with proper care. The single biggest factor determining where your cat falls on that spectrum is the type of infection their immune system mounts against the virus.
Progressive vs. Regressive Infection
When a cat is exposed to FeLV, their immune system responds in one of a few ways, and this response essentially sets the trajectory for everything that follows. Roughly 30 to 40% of exposed cats develop what’s called a progressive infection, where the virus establishes itself in the bone marrow and replicates continuously. These cats shed viral particles, can infect other cats, and face the highest risk of fatal complications. In one Brazilian study tracking FeLV-positive cats, those with progressive infections had a median survival of just 30 days after diagnosis.
Another 30 to 40% of cats mount a partially effective immune response and develop a regressive infection. In these cats, the virus gets incorporated into their DNA but the immune system keeps it from actively replicating. They don’t shed virus, they’re very unlikely to show clinical signs, and their survival times are dramatically better. In the same study, the median survival for cats with regressive infections was never reached during the follow-up period, meaning most of them were still alive when the study ended. Over time, more than half of regressed cats tested negative on both blood and DNA-based tests, suggesting their immune systems had effectively cleared the infection.
Kittens are at much higher risk of developing progressive disease than adult cats exposed to the same virus. A mature immune system is simply better equipped to fight FeLV into regression, which is one reason age at infection matters so much for long-term outlook.
What Determines How Long Your Cat Lives
Beyond infection type, several practical factors influence survival. A large Italian study found that FeLV-positive cats had a median survival of about 714 days (just under two years), compared to nearly 11 years for cats without retroviral infections. Cats co-infected with both FeLV and feline immunodeficiency virus fared far worse, with a median survival of only 77 days.
The complications that shorten life in progressive FeLV are the ones you’d expect from a virus that suppresses the immune system and can trigger abnormal cell growth. Lymphoma (a cancer of white blood cells), severe anemia, and secondary infections are the most common threats. Because the virus lives in bone marrow, it disrupts the production of healthy blood cells, leaving the cat vulnerable on multiple fronts at once.
Reactivation is also possible in cats with regressive infections. If the cat becomes immunosuppressed through another illness or certain medications, the dormant virus can begin replicating again. This is uncommon but means that even cats who appear to have cleared the virus still carry some long-term risk.
How Indoor Care Extends Survival
The gap between average survival and best-case survival largely comes down to management. FeLV-positive cats kept strictly indoors with consistent veterinary monitoring often survive four to five years after diagnosis, and some live longer. Outdoor living, by contrast, exposes cats to fights, parasites, and secondary infections that accelerate decline.
Keeping your cat indoors does three things at once: it prevents them from spreading FeLV to other cats, it shields them from pathogens their weakened immune system can’t handle, and it reduces physical stress. A high-quality diet rich in antioxidants, regular parasite control, and a low-stress environment all contribute to keeping the immune system as functional as possible. Veterinary check-ups every three to four months allow early detection of anemia, weight loss, or other signs that the virus is becoming more active.
What Test Results Tell You About Prognosis
If your cat has been diagnosed through a standard blood screening test (ELISA), it’s worth understanding what the result means for prognosis. A positive ELISA detects viral proteins circulating in the blood and confirms exposure, but some cats who test positive on ELISA will later clear the virus from their bloodstream. A second type of test (IFA) detects the virus in white blood cells and indicates bone marrow infection. Cats that test positive on IFA are almost always persistently infected and unlikely to clear the virus. If your cat tested positive on ELISA alone, a retest in 30 to 60 days can help clarify whether the infection is progressive or regressive.
Treatment Options and Their Limits
There is no cure for FeLV, but treatment can improve quality of life and modestly extend survival. One well-studied approach involves a type of antiviral immune protein. In a controlled trial, treated cats had a mortality rate of 39% at nine months compared to 59% in untreated cats, representing a meaningful but not dramatic improvement. By 12 months, the gap narrowed (47% vs. 59%), suggesting the benefit was strongest in the earlier months of treatment.
Most management focuses on supportive care: treating secondary infections promptly, managing anemia with nutritional support or transfusions when needed, and monitoring for early signs of lymphoma. The goal is to keep the cat comfortable and functional for as long as possible. Many FeLV-positive cats live good-quality lives for years, especially those whose immune systems manage to keep the virus in check.
