Cats managed on daily medication (methimazole or carbimazole) have a somewhat shorter median survival of around 14 months in one study, though this number comes with important context. Cats placed on medication tend to be older, sicker, or have other health problems that made their owners opt against a permanent fix. The medication itself doesn’t shorten life. It simply reflects a different population of cats.
Surgery (thyroidectomy) can also be curative, but outcomes vary more widely. In one study of 95 cats, 60% stayed in remission after surgery while 40% had their hyperthyroidism return. A longer-term study following 23 cats for up to five years found that 44% eventually developed recurrent hyperthyroidism. How Kidney Disease Changes the Picture
This is the complication that matters most for prognosis, and it’s frustratingly common. Hyperthyroidism and chronic kidney disease frequently coexist in older cats, and they interact in a tricky way. The overactive thyroid actually increases blood flow to the kidneys, which can mask underlying kidney disease by making the kidneys appear to function better than they really do. Once you treat the hyperthyroidism and thyroid levels normalize, kidney problems that were hidden may surface.
Cats with pre-existing kidney problems before treatment have a less favorable prognosis overall. However, there’s a reassuring finding: cats that develop signs of kidney trouble only after their hyperthyroidism is treated appear to do about as well as cats whose kidneys stay normal. Your vet will monitor kidney values closely in the weeks after starting treatment, and the treatment plan may need adjusting based on what those numbers show.
Factors That Influence Your Cat’s Outlook
Beyond treatment choice, several things tip the odds in your cat’s favor or against it:
- Age at diagnosis. A cat diagnosed at 10 has more potential years ahead than one diagnosed at 16, simply because of normal aging. Most hyperthyroid cats are seniors already.
- Severity at diagnosis. Cats caught early, often through routine bloodwork before symptoms become obvious, tend to respond better and avoid heart damage.
- Heart involvement. If the thyroid has already caused thickening of the heart muscle, treatment can reverse some of that damage, but not always completely. Cats without cardiac changes at diagnosis do better long term.
- Consistency of treatment. For cats on medication, missed doses mean uncontrolled thyroid levels and ongoing organ damage. Medication needs to be given for the rest of the cat’s life, typically twice daily.

The prescription diet costs roughly what premium cat food does, but the restriction that your cat can eat absolutely nothing else makes it impractical for many households, especially those with multiple cats.

If your cat was just diagnosed and is otherwise healthy, treatment gives you a reasonable expectation of 2 to 4 more years together, with some cats living well beyond that. Cats treated with radioactive iodine and caught before significant heart or kidney damage have the best statistical odds. Cats on well-managed medication do well too, particularly when their owners stay consistent with dosing and follow-up bloodwork.
The diagnosis itself is not a death sentence. Hyperthyroidism is one of the most treatable conditions in feline medicine. The cats that do poorly are generally the ones diagnosed late, left untreated, or dealing with significant kidney disease alongside the thyroid problem. Early detection through routine senior bloodwork, ideally starting around age 7 to 10, gives your cat the best chance at a long, comfortable life after diagnosis.
