How Long Can a Dog Live With Lung Cancer: Survival Times

A dog’s survival time with lung cancer depends heavily on the type of cancer, how early it’s caught, and whether it has spread. Dogs with early-stage primary lung cancer that’s surgically removed can live around two to three years or longer, while dogs with advanced or metastatic disease may survive only weeks to a few months. The range is wide, so understanding what drives these differences can help you know what to expect for your dog.

Primary vs. Metastatic Lung Cancer

The first distinction that shapes your dog’s prognosis is whether the cancer started in the lungs or arrived there from somewhere else. Primary lung cancer originates in the lung tissue itself, most commonly as a type called pulmonary adenocarcinoma. This is relatively uncommon in dogs but tends to have better treatment options.

Metastatic lung cancer means a tumor elsewhere in the body, such as in the bones, mammary glands, or bladder, has spread to the lungs. This form carries a worse prognosis and fewer treatment options than primary lung cancer, because it signals the disease has already progressed significantly. When vets discover a lung mass, they’ll typically recommend bloodwork and an abdominal ultrasound to determine which type your dog has.

Survival Times by Stage

For primary lung cancer, staging is the strongest predictor of how long a dog will live after surgery. A study of 71 dogs that had their lung tumors surgically removed found dramatic differences across stages:

  • Stage I: median survival of 952 days (about 2.6 years)
  • Stage II: median survival of 658 days (about 1.8 years)
  • Stage III: median survival of 158 days (about 5 months)
  • Stage IV: median survival of 52 days (under 2 months)

Staging accounts for tumor size, whether the cancer has reached nearby lymph nodes, and whether it has spread to other parts of the body. The jump between Stage II and Stage III is steep, dropping from nearly two years to just five months. This is largely because Stage III and IV cancers have typically invaded surrounding structures or spread beyond the lung.

How Tumor Size Affects Prognosis

Tumor size at the time of surgery is one of the most practical indicators your vet can measure. A study of small-breed dogs with lung adenocarcinoma found that smaller tumors translated directly into longer survival:

  • Tumors 3 cm or smaller: median survival over 1,064 days (nearly 3 years)
  • Tumors 3–5 cm: median survival of 704 days (about 2 years)
  • Tumors 5–7 cm: median survival of 574 days (about 1.5 years)
  • Tumors larger than 7 cm: median survival of just 110 days (under 4 months)

The overall median survival for the dogs in this study was 716 days, roughly two years. These numbers apply to dogs that had surgery, so they represent a best-case treatment scenario. Dogs whose tumors are caught when they’re still small clearly do much better, which is one reason vets emphasize follow-up when a suspicious shadow appears on a chest X-ray taken for another reason.

Tumor Grade Matters as Much as Size

Beyond how big the tumor is, how aggressive its cells look under a microscope plays a major role. Pathologists grade lung tumors on a scale from well-differentiated (Grade 1, meaning the cells still resemble normal lung tissue) to poorly differentiated (Grade 3, meaning the cells are highly abnormal and fast-growing).

The survival differences are stark. Dogs with well-differentiated tumors had a median survival of 790 days, over two years. Dogs with moderately differentiated tumors survived a median of 251 days, about eight months. Dogs with poorly differentiated tumors had a median survival of just 5 days, essentially meaning most did not recover from the point of diagnosis or surgery. This grading information comes from the pathology report after a biopsy or surgical removal, and it’s one of the most important numbers to ask your vet about.

Lymph Node Involvement

Whether cancer has reached the lymph nodes near the lungs is another critical dividing line. Dogs whose lymph nodes test negative for cancer at the time of surgery have a median survival of roughly 285 to 452 days. Dogs with lymph node involvement drop to a median of 26 to 167 days. If your vet mentions that the lymph nodes looked suspicious on imaging or came back positive on biopsy, this significantly changes the expected timeline.

Surgery and Chemotherapy Outcomes

Surgery to remove the affected lung lobe is the primary treatment for dogs with primary lung cancer, and it offers the best survival outcomes when the cancer hasn’t spread. Dogs treated with surgery alone in one study had a median survival of 568 days, roughly 19 months.

Perhaps surprisingly, adding chemotherapy after surgery did not improve outcomes in the studies available. Dogs receiving surgery plus chemotherapy had a median survival of 361 days, and their cancer progressed faster, with a median time to progression of 143 days compared to 427 days for surgery alone. Multiple studies have found no statistically significant survival benefit from adjuvant chemotherapy for primary lung cancer in dogs. This doesn’t mean chemotherapy is never appropriate, as individual cases vary, but it does mean surgery alone remains the standard when complete tumor removal is possible.

For dogs that aren’t candidates for surgery due to the tumor’s location, spread, or other health issues, options are more limited. Palliative care focused on comfort, including pain management and medications to ease breathing, becomes the priority.

Signs That a Dog’s Condition Is Declining

Many dogs with lung cancer show few symptoms early on. A chronic cough is often the first noticeable sign, sometimes accompanied by reduced energy or mild exercise intolerance. As the disease advances, symptoms become harder to miss.

In the later stages, dogs typically show labored or rapid breathing, persistent coughing that may produce blood, significant weight loss, loss of appetite, and increasing lethargy. Some dogs become reluctant to lie down because certain positions make breathing harder. When these symptoms can no longer be managed with medication, or when your dog stops engaging in activities they used to enjoy, eating, greeting you at the door, wanting to go outside, it generally signals the cancer is overwhelming their body’s ability to cope.

Quality of life, rather than a specific number of days, is what most veterinary oncologists use to guide end-of-life decisions. Tracking daily patterns like appetite, mobility, breathing comfort, and engagement with the family can help you and your vet recognize when that quality is slipping.

What Shapes Your Dog’s Individual Outlook

No single number from a study can tell you exactly how long your dog will live. Median survival means half the dogs lived longer and half lived shorter. Your dog’s outcome depends on a combination of factors: the cancer’s stage, tumor size, grade, lymph node status, whether complete surgical removal was possible, and your dog’s overall health.

Dogs diagnosed early with a small, well-differentiated tumor that’s fully removed by surgery have the best chance, potentially living three years or more. Dogs diagnosed later with large, aggressive tumors or lymph node involvement face a much shorter timeline, often measured in weeks to a few months. Asking your vet specifically about the stage, grade, and lymph node findings from your dog’s case gives you the most accurate picture of where your dog falls in that range.
