What makes this harder to pin down is that warning signs can appear long before the main event. Early symptoms, like brief episodes of chest tightness, unusual fatigue, or shortness of breath, can come and go for hours, days, or even weeks beforehand. These episodes might last five or ten minutes and then disappear entirely, only to return days later when the full heart attack occurs. What’s happening inside your heart follows a surprisingly tight schedule. Within seconds of a coronary artery becoming blocked, heart muscle cells start changing at a microscopic level. After about 30 minutes, the injury starts becoming permanent. Cells begin to die, and that dead tissue can never regenerate. The damage doesn’t stop all at once at the three-hour mark, though. It typically lasts 5 to 10 minutes and eases when you rest or the trigger stops. A heart attack, by contrast, lasts longer than 10 minutes and doesn’t respond to rest. Silent Heart Attacks Can Go Unnoticed for Months

People talk themselves out of it, attribute the pain to something else, or wait to see if it passes on its own.
That delay is costly. Heart muscle that could have been saved in the first hour is dead by hour four. The treatments used to reopen blocked arteries, whether through a catheter-based procedure or clot-dissolving medication, work best when performed as early as possible. Outcomes drop significantly with every passing hour. A heart attack that’s treated in the first 60 to 90 minutes can result in minimal lasting damage. The same heart attack treated six hours later can leave a person with permanent heart failure.
What Recovery Looks Like After the Acute Phase
Once the artery is reopened and the acute crisis passes, your body begins repairing the damaged area. Proteins released by dying heart cells, called troponins, spike in the bloodstream within 3 to 12 hours and peak around 24 hours after the event. Doctors use serial blood tests tracking these levels to confirm the diagnosis and gauge how much damage occurred.
The heart muscle itself takes weeks to heal. Dead tissue is gradually replaced by scar tissue, which doesn’t contract the way healthy muscle does. How much scar tissue forms depends almost entirely on how long the artery was blocked before treatment. A small area of scarring may cause no noticeable symptoms.
