At 24 hours, you’ll feel tired and foggy, but you can still function. Your alertness, attention span, and decision-making all take a measurable hit. Most people have pulled an all-nighter at some point and recognize this level of impairment: you’re slower, clumsier, and more prone to mistakes, but you’re still coherent.

At 48 hours, things shift. Your brain starts producing involuntary microsleeps, brief episodes lasting just a few seconds where you essentially lose consciousness without realizing it. Anxiety, irritability, and a strange sense of detachment from yourself (sometimes called depersonalization) can set in. Staying awake becomes an active struggle rather than just uncomfortable.

At 72 hours, perception starts to break down in serious ways. Hallucinations become more frequent and more complex. Disordered thinking and delusions can appear. Your urge to sleep at this point may become nearly impossible to resist without external help.

Beyond 96 hours, the distortion of reality can resemble acute psychosis. People at this stage report elaborate visual and auditory hallucinations, paranoid thinking, and a near-total collapse of logical reasoning. The drive to sleep becomes physically overwhelming.

When Hallucinations Start

Simple hallucinations, like seeing flickers of movement in your peripheral vision, can appear as early as 24 to 36 hours without sleep. But full, complex hallucinations (seeing things that aren’t there, hearing voices) typically don’t develop until the three- to four-day mark. The timing varies by person. Some people are more resilient, while others begin experiencing perceptual distortions earlier, especially under stress or in dim environments.

How Your Body Responds Internally

The psychological effects are the most visible signs, but sleep deprivation also disrupts systems you can’t feel directly. A University of Chicago study found that after building up a significant sleep debt, subjects took 40% longer than normal to regulate blood sugar after a high-carbohydrate meal. The danger comes indirectly, from accidents, impaired judgment, or the physical toll of prolonged stress on the heart and metabolism.

There is, however, one condition that proves sleep deprivation can be fatal. Fatal familial insomnia is a rare genetic prion disease that progressively destroys the part of the brain controlling sleep, a region called the thalamus. People with this condition gradually lose the ability to sleep at all. Death typically occurs 7 to 73 months after symptoms begin, though the cause of death involves widespread neurological collapse rather than sleeplessness in isolation. It remains the strongest evidence that the body cannot survive indefinitely without sleep.

Why the Record Will Likely Stand

After Gardner’s 1964 experiment, a few people attempted to push the boundary further. Robert McDonald reportedly stayed awake for 453 hours, nearly 19 days. But Guinness World Records eventually stopped accepting new attempts in this category altogether, citing safety concerns. No major research institution has attempted to replicate extreme sleep deprivation experiments either, because the psychiatric risks are too significant to justify.

Gardner himself recovered fully after sleeping about 14 hours following his experiment, though he later reported struggling with insomnia for years afterward. His case showed that the brain can survive 11 days without sleep, but not without serious temporary consequences, and not without the body fighting you every step of the way.
