Lantus (insulin glargine) can be kept unrefrigerated for up to 28 days, as long as the temperature stays below 86°F (30°C). After 28 days at room temperature, you should discard it, even if insulin remains in the vial or pen. This applies to both opened and unopened Lantus, though the specific handling rules differ slightly between vials and pens.
Storage Rules for Vials vs. Pens
Lantus comes in two forms, and each has slightly different storage instructions once you start using it.
The 10 mL multi-dose vial can be stored either in the refrigerator or at room temperature once opened. Either way, it’s good for 28 days from the date you first puncture it. Before opening, keep it refrigerated between 36°F and 46°F (2°C and 8°C), where it stays good until the printed expiration date.
The SoloStar prefilled pen has one key difference: once opened, it should be kept at room temperature only. Do not put an in-use pen back in the refrigerator. Like the vial, it lasts 28 days after first use. Unopened pens stay good in the fridge until the expiration date.
What “Room Temperature” Actually Means
The 28-day window assumes the insulin stays below 86°F (30°C). That’s an important number, because room temperature in a hot apartment, a parked car, or a bag sitting in the sun can easily exceed it. At higher temperatures, the insulin breaks down faster and may lose potency well before the 28 days are up, with no visible warning signs.
Direct heat and light also degrade Lantus. The manufacturer specifically warns to protect both vials and pens from direct sunlight and heat sources. Even if the ambient temperature feels fine, leaving a pen on a windowsill or dashboard can push the insulin past safe thresholds quickly.
How to Tell if Lantus Has Gone Bad
Lantus is normally a clear, colorless solution. If you notice cloudiness, floating particles, or crystals inside the vial or pen cartridge, the insulin has degraded and should be thrown away. That said, insulin doesn’t always change in appearance when it loses potency. It can look perfectly normal and still be less effective, especially after prolonged heat exposure or use past the 28-day mark.
The most reliable indicator is often your blood sugar readings. If your glucose levels start running higher than usual and you haven’t changed your diet, activity, or dose, compromised insulin is worth considering as a cause.
Freezing Is Worse Than Warmth
While heat gradually reduces potency, freezing destroys insulin outright. The FDA and the manufacturer are both clear: never use Lantus that has been frozen, even if it thaws and looks normal. Freezing damages the protein structure in a way that can’t be reversed. If your insulin was stored near ice packs, in a freezer-adjacent refrigerator shelf, or in checked luggage in a cargo hold, check that it hasn’t iced over before using it.
Keeping Lantus Safe During Travel
Travel is where storage rules get tricky. Checked luggage in an airplane cargo hold can freeze, and a bag left in a hot car can easily exceed 86°F. A few practical options can help.
- Evaporative cooling cases: Products like Frio cases use water-activated cooling to keep insulin between 64°F and 79°F for 45 hours or more, even when the outside temperature reaches 100°F. They don’t require electricity or ice.
- Insulated coolers with cold packs: USB-powered or cold-pack coolers can maintain refrigerator-like temperatures for 24 to 72 hours depending on the product. If you use ice or cold packs, make sure the insulin isn’t in direct contact with them to avoid accidental freezing.
- Wireless temperature monitors: Small Bluetooth sensors can track the temperature inside your case and alert you through an app if it drifts out of range.
Always carry insulin in your carry-on bag when flying. Cargo holds aren’t temperature-controlled and can swing between extremes. TSA allows insulin and related supplies through security, including cooling equipment.
What Happens if You Used Expired or Overheated Lantus
Using Lantus that sat out longer than 28 days or was exposed to high heat won’t cause a toxic reaction. The risk is that the insulin has lost some or all of its blood-sugar-lowering ability, which means your glucose could spike. If you suspect your insulin was compromised, replace it with a fresh vial or pen and monitor your blood sugar more closely for the next day or two to make sure your levels return to your usual range.
