Most people with a leaky heart valve live for many years, and many reach a normal lifespan. The answer depends heavily on which valve is leaking, how severe the leak is, whether you have symptoms, and whether the valve is eventually repaired or replaced. A mild leak in someone with no symptoms can remain stable for decades, while a severe untreated leak that causes heart failure symptoms carries a median survival under two years.
Severity Is the Biggest Factor
Heart valve leaks are graded as mild, moderate, or severe based on how much blood flows backward through the valve with each heartbeat. Mild leaks are extremely common, especially as people age, and most never progress to the point of causing problems. In a large population study, only about 1.3% of people with moderate valve disease progressed to severe disease over a roughly three-year follow-up period. The odds of progression increase with age: people in their 60s progress at roughly twice the rate of those in their 50s, and those in their 70s progress faster still.
Severe leaks are a different story. A study published in JACC: Cardiovascular Imaging tracked patients with severe mitral valve leaks who had no symptoms and were being monitored without surgery. Their survival was 94.6% at six years, 85.6% at ten years, and 74.5% at fifteen years. Those are strong numbers for a condition labeled “severe,” and they reflect the fact that even a significant leak can be managed for years when the heart is still compensating well.
Which Valve Makes a Difference
Your heart has four valves, and leaks in each one carry different risks.
The mitral valve (between the left atrium and left ventricle) is the most commonly affected. Severe mitral regurgitation has the survival numbers described above. Many people live 10 to 15 years or more with close monitoring, and outcomes improve further when the valve is repaired or replaced at the right time.
The aortic valve tends to be more dangerous when it becomes severely leaky and symptoms appear. Once a person with severe aortic regurgitation develops heart failure symptoms like breathlessness at rest or significant fluid retention, median survival without treatment drops below two years. The key distinction is that people can have severe aortic leaks for years before symptoms start. During that asymptomatic phase, the outlook is much better.
The tricuspid valve (on the right side of the heart) is often overlooked, but severe leaks here carry real risk. One-year survival drops from about 92% with no tricuspid leak to 64% with a severe one. Tricuspid leaks often develop alongside other heart conditions, which partly explains the lower survival numbers.
Pulmonary valve leaks (the valve between the right ventricle and the lungs) are the least common and generally the most benign. Mild pulmonary regurgitation is considered a normal finding in many people.
Symptoms Signal a Turning Point
The single most important milestone in living with a leaky valve is the onset of symptoms. Before symptoms appear, your heart compensates by pumping harder or enlarging slightly to handle the extra volume of blood flowing backward. This compensation can work for years.
When it stops working, you’ll notice shortness of breath during activity, fatigue that limits what you can do, swelling in your ankles or legs, or a fluttering sensation in your chest. The worse your symptoms, the higher your risk. Patients with more severe functional limitations, meaning they struggle with even light daily activities, face progressively higher rates of death and hospitalization compared to those whose symptoms only show up during vigorous exercise.
This is why doctors monitor leaky valves with regular echocardiograms even when you feel fine. The goal is to catch the moment when the heart starts to strain, sometimes before you notice symptoms yourself, so treatment can happen before permanent damage sets in.
How Repair or Replacement Changes the Timeline
Valve surgery dramatically changes the survival equation. For the mitral valve specifically, patients under 75 who underwent valve repair had survival rates of 93.6% at one year, 83.3% at five years, and 66.4% at ten years. Repair consistently outperforms replacement: patients under 75 who had their valve replaced instead of repaired saw ten-year survival drop to 47.4%.
Age at the time of surgery matters significantly. Patients 75 and older who underwent mitral valve repair had ten-year survival of 40.3%, compared with 66.4% for younger patients. For valve replacement in that older group, ten-year survival was 28.1%. These numbers reflect the reality that older patients have more health conditions competing for their longevity, not just the valve disease itself.
For aortic valve replacement using a tissue (bioprosthetic) valve, a large study of patients aged 60 and older found median survival ranged widely based on overall health. Low-risk patients survived a median of 10.9 years after surgery, intermediate-risk patients 7.3 years, and high-risk patients 5.8 years. Among low-risk patients aged 60 to 64, median survival reached 16.2 years, while those 85 and older had a median of 6.1 years. These post-surgical lifespans approach what you’d expect for the general population at those ages.
Living With a Mild or Moderate Leak
If you’ve been told you have a mild or even moderate valve leak, the outlook is reassuring. Most mild leaks never progress to anything dangerous. You’ll typically need an echocardiogram every one to three years so your doctor can track whether the leak is stable or worsening. Between appointments, there’s usually no restriction on normal activities.
Keeping blood pressure well controlled reduces the workload on a leaky valve and slows progression. Staying physically active, maintaining a healthy weight, and managing conditions like high blood pressure or diabetes all contribute to keeping a mild leak from becoming a bigger problem. Many people with mild regurgitation live their entire lives without ever needing surgery.
What Determines Whether a Leak Gets Worse
Several factors influence whether your leak will stay stable or progress over time. Age is the strongest predictor. In population data, progression rates climbed from about 6% in people in their 50s to 25% in those in their 70s, measured over roughly three years. The underlying cause also matters. A leak caused by a degenerative condition like mitral valve prolapse may worsen gradually over decades. A leak caused by a heart attack or infection can worsen much more quickly.
Heart size is another signal doctors watch closely. As the heart enlarges to compensate for the backward flow, it eventually reaches a point where the stretching itself makes the leak worse, creating a cycle that accelerates decline. Echocardiograms track the dimensions of your heart chambers for exactly this reason. A sudden increase in chamber size, even without new symptoms, can prompt a conversation about surgery.
Atrial fibrillation, an irregular heart rhythm, is both a consequence and an accelerator of valve leaks. When a leaky valve causes the left atrium to enlarge, atrial fibrillation becomes more likely, and once it starts, it adds further strain to an already compromised system.
The Bottom Line on Lifespan
A mild leaky valve is compatible with a normal lifespan. A severe leaky valve that’s caught before symptoms develop and treated at the right time still allows many people to live 10 to 20 additional years or more, depending on age and overall health. The danger zone is a severe leak that goes undiagnosed or untreated after symptoms begin, where survival drops sharply within just a few years. Regular monitoring is the bridge between a manageable condition and a life-threatening one.
