How long you can live with cirrhosis depends heavily on whether the disease has caused visible complications. People with compensated cirrhosis, meaning the liver is scarred but still functioning well enough to avoid symptoms, have a median survival of more than 12 years. Once complications develop, that number drops sharply to around 2 years. But these are medians, not ceilings. Many people live well beyond them, especially with the right management.

Compensated vs. Decompensated Cirrhosis

The single most important factor in cirrhosis life expectancy is which stage you’re in. Doctors divide cirrhosis into two broad categories based on whether the liver can still keep up with its essential jobs.

Compensated cirrhosis means the liver has significant scarring but hasn’t yet failed in a way that produces noticeable problems. You might feel fatigued or have some lab abnormalities, but the liver is compensating for the damage. At this stage, median survival exceeds 12 years, and many people live considerably longer, particularly if the underlying cause of the scarring is identified and treated.

Decompensated cirrhosis is the turning point. It’s defined by the appearance of at least one major complication: fluid buildup in the abdomen (ascites), yellowing of the skin and eyes (jaundice), bleeding from swollen veins in the esophagus, or confusion caused by toxins the liver can no longer filter (hepatic encephalopathy). Once any of these occur, median survival drops to roughly 2 years. The transition from compensated to decompensated cirrhosis happens at a rate of about 5% to 7% per year, so staying compensated for as long as possible is the central goal of treatment.

How Severity Scores Predict Survival

Doctors use two main scoring systems to estimate how much liver function remains and what that means for your prognosis. Neither one is a crystal ball, but both give a useful picture.

The Child-Pugh score groups cirrhosis into three classes based on lab results and symptoms. Class A represents the mildest disease, with essentially 100% one-year survival and 85% two-year survival. Class B is intermediate: about 80% survive one year and 60% survive two years. Class C is the most severe, with one-year survival around 45% and two-year survival around 35%.

The MELD score (Model for End-Stage Liver Disease) is a number calculated from blood tests that reflects how urgently someone needs a liver transplant. It also predicts short-term mortality. A MELD score between 10 and 19 carries a 3-month mortality risk of roughly 1% to 4%. At 20 to 29, that risk climbs to about 5% to 20%. Scores of 30 to 39 correspond to 12% to 48% three-month mortality, and scores at or above 40 carry a 25% to 60% chance of dying within three months. If your doctor mentions your MELD score, it’s worth asking what range you fall into and what that means for your timeline.

What Causes the Differences

Not all cirrhosis progresses at the same speed. The underlying cause matters. Cirrhosis driven by hepatitis C historically carried one of the higher risks of complications and liver cancer, though today’s antiviral treatments can eliminate the virus entirely and slow further damage. Alcohol-related cirrhosis has a highly variable trajectory: people who stop drinking can stabilize or even partially reverse early-stage scarring, while those who continue drinking face rapid progression. Cirrhosis from fatty liver disease (now called metabolic dysfunction-associated steatotic liver disease) tends to progress more slowly in many cases, but carries its own set of metabolic risks.

Regardless of cause, people with cirrhosis face an ongoing risk of developing liver cancer. The annual incidence is about 2.4% across all causes, meaning roughly 1 in 40 people with cirrhosis develop liver cancer each year. That risk is highest with hepatitis C (about 3% per year) and alcohol-related disease (about 2.7%) and somewhat lower with fatty liver disease (about 2%). This is why regular screening with ultrasound every six months is standard for anyone with cirrhosis.

How Liver Transplant Changes the Picture

For people with decompensated cirrhosis or liver cancer, transplant offers the possibility of a near-normal lifespan. Current data from the U.S. transplant registry shows a one-year survival rate of 93.5% after liver transplant and an 81% five-year survival rate. At ten years, about 65% of transplant recipients are still alive. These are remarkable numbers for a population that, without transplant, would face a median survival measured in months to a few years.

The catch is availability. Not everyone qualifies, and wait times vary dramatically by region and blood type. Your MELD score determines your place on the waiting list, with higher scores (meaning sicker patients) getting priority. For people with alcohol-related cirrhosis, most transplant centers require a period of sustained sobriety, typically six months, before listing.

What You Can Do to Live Longer

The actions that extend life with cirrhosis aren’t dramatic. They’re consistent, daily choices that protect the liver function you still have.

If alcohol caused or contributed to your cirrhosis, complete abstinence is the single most powerful intervention. Even in decompensated disease, stopping alcohol can improve liver function enough to shift someone from one severity class to a better one. If a virus caused the damage, treating and clearing it (hepatitis C is now curable; hepatitis B can be suppressed long-term) prevents further scarring and reduces cancer risk.

Nutrition is surprisingly important and often overlooked. Muscle wasting, called sarcopenia, is common in cirrhosis and is independently linked to higher complication rates and shorter survival. The American Association for the Study of Liver Diseases recommends that people with stable cirrhosis eat 1.2 to 1.5 grams of protein per kilogram of ideal body weight each day. That’s significantly more than the standard recommendation for healthy adults. If you already have muscle loss, the target is 1.5 grams per kilogram. Spreading protein intake across meals and including a late-evening snack helps prevent the overnight fasting that accelerates muscle breakdown in cirrhosis.

Staying on top of screening and follow-up matters too. Regular lab work tracks how your liver function is trending. Ultrasound every six months catches liver cancer early, when it’s most treatable. Endoscopy can identify dangerous vein swelling in the esophagus before it bleeds. These aren’t just bureaucratic check-ins. Each one is an opportunity to catch a problem while it’s still manageable, which is the difference between staying compensated and tipping into decompensation.

What the Numbers Actually Mean for You

Statistics like “median survival of 2 years” can feel like a sentence, but they describe populations, not individuals. Half of all people in that group live longer than 2 years, some much longer. Your personal trajectory depends on how well you respond to treatment, whether the underlying cause can be controlled, your nutritional status, your age, and whether you develop additional complications like infection or kidney problems.

People diagnosed with compensated cirrhosis in their 40s or 50s who address the root cause, maintain good nutrition, and avoid further liver insults can realistically live 15 to 20 years or more. People diagnosed with decompensated cirrhosis who receive a transplant can expect decades of additional life. Even those who aren’t transplant candidates can often extend their survival well beyond the median with aggressive management of complications like fluid retention and encephalopathy.

The most important question isn’t “how long do I have?” but “what stage am I in, and what can I do right now to stay there or improve?” That conversation with your liver specialist is where the real answers live, because the numbers shift based on every decision you make from this point forward.
