There is no fixed time limit for taking meloxicam 15 mg. The FDA does not specify a maximum duration, but its guidance is clear: use the lowest effective dose for the shortest duration that meets your treatment goals. Many people take meloxicam for months or even years to manage chronic conditions like osteoarthritis or rheumatoid arthritis, but longer use comes with increasing risks that need to be weighed and monitored.
Why There’s No Set Time Limit
Meloxicam is prescribed for conditions that don’t go away. Osteoarthritis, for example, is a lifelong condition, and some people need ongoing pain relief to function. The FDA approves meloxicam for these uses without capping the duration because the decision depends on each person’s pain level, risk factors, and response to the drug. Clinical trials have studied meloxicam use for up to one year, and many prescribers continue it well beyond that when the benefits justify it.
That said, every major guideline, from the FDA to the American College of Rheumatology, repeats the same principle: the shortest duration possible. This isn’t a vague suggestion. It reflects real evidence that risks accumulate the longer you stay on any NSAID, meloxicam included.
What Happens in Your Body Over Time
Meloxicam reaches its full steady level in your bloodstream by day five of daily dosing. From that point on, the drug is consistently active, reducing inflammation and pain. But it’s also consistently affecting your stomach lining, kidneys, and cardiovascular system.
In clinical trials lasting up to one year, about 25% of patients on meloxicam developed gastrointestinal side effects, and roughly 5% experienced a serious adverse event. Around 5% of patients stopped taking the drug because of side effects. These numbers were actually lower than those seen with naproxen in the same trials (where about 31% had GI problems and over 8% discontinued), which is one reason meloxicam is considered relatively well tolerated among NSAIDs. But “relatively well tolerated” still means one in four people has stomach-related issues over a year of use.
Cardiovascular Risk Increases With Duration
The most serious concern with long-term meloxicam use is cardiovascular. All NSAIDs carry a boxed warning (the FDA’s strongest) stating they increase the risk of heart attack and stroke, and that this risk may grow with duration of use. Some studies have found this elevated risk begins within the first few weeks of treatment.
For most people without heart disease, the absolute risk remains small. But if you have existing cardiovascular disease or risk factors like high blood pressure, diabetes, or high cholesterol, the baseline risk is already elevated, and meloxicam adds to it. People who have had a recent heart attack face a notably higher danger: observational data from a large Danish registry found that NSAID users in the first year after a heart attack had a death rate of 20 per 100 person-years, compared to 12 per 100 person-years in those not taking NSAIDs.
Higher doses also carry more cardiovascular risk. If you’re on 15 mg (the maximum daily dose), this is worth discussing with your prescriber, especially if you’ve been on it for a while. Dropping to 7.5 mg, if it still controls your symptoms, meaningfully reduces your exposure.
Stomach, Kidney, and Liver Concerns
Meloxicam can cause stomach ulcers and GI bleeding, sometimes without any warning symptoms beforehand. The risk is dose-dependent but remains relatively low at both the 7.5 mg and 15 mg doses compared to older NSAIDs. Still, several factors stack the odds against you: being over 65, drinking alcohol, smoking, taking blood thinners or corticosteroids, or having a history of ulcers.
Your kidneys are also affected. Meloxicam reduces blood flow to the kidneys, which is usually harmless in healthy, well-hydrated people but can cause problems if you’re dehydrated, have existing kidney disease, or take blood pressure medications like ACE inhibitors or ARBs. The combination of meloxicam with these blood pressure drugs, particularly in older adults, requires periodic kidney function checks.
Because serious GI bleeding, liver damage, and kidney injury can develop without obvious symptoms, the FDA recommends that people on long-term NSAID therapy get periodic blood work, including a complete blood count and a basic chemistry panel that checks kidney and liver function.
What Long-Term Use Should Look Like
If you and your prescriber decide that ongoing meloxicam is the right call, a few things make it safer. First, regular check-ins matter. Blood work every few months catches kidney or liver changes before they become serious. Second, periodically testing whether you still need the full 15 mg dose is worthwhile. Some people find that after inflammation stabilizes, 7.5 mg is enough, or that they can take breaks from the drug during lower-pain periods.
Avoid stacking meloxicam with other NSAIDs, including over-the-counter ibuprofen or naproxen. The general rule is one NSAID at a time. If you take low-dose aspirin for heart protection, know that combining it with meloxicam increases bleeding risk and may require closer monitoring. The same applies if you’re on blood thinners, certain antidepressants (SSRIs), or oral steroids.
Stopping Meloxicam After Long-Term Use
Meloxicam is not addictive, and there is no formal withdrawal syndrome. However, stopping abruptly after months or years of use can lead to a noticeable return of pain and inflammation, sometimes called rebound symptoms. This isn’t a sign of dependence; it simply means the underlying condition is still active and was being masked by the drug.
Some prescribers recommend tapering gradually rather than stopping all at once, which gives your body time to adjust. If you’re planning to stop, it helps to have an alternative pain management strategy in place, whether that’s physical therapy, a different class of medication, or topical treatments.
