But the full picture is more nuanced. How long the shot actually “lasts” depends on whether you’re asking about the initial spike in your bloodstream, the refill to your body’s storage, or how long you’ll feel the benefits before symptoms creep back.

What Happens After the Injection

B12 from an intramuscular injection is absorbed rapidly, reaching peak levels in your blood within about one hour. Your body takes what it needs and starts filtering the excess through your kidneys almost immediately, with a significant portion excreted in the first eight hours. That bright yellow urine you notice after a shot is largely unused B12 leaving your body.

Not all of the dose gets flushed out, though. The form of B12 matters here. That said, some research finds the practical differences in bioavailability between forms are small, with factors like age and genetics playing a bigger role.

The real story of how long B12 lasts isn’t about what’s circulating in your blood on any given day. It’s about what’s stored in your liver. Your body typically holds between 1 and 5 milligrams of B12 in reserve, which is 1,000 to 2,000 times the amount you need daily. Each injection tops off that reservoir.

In a healthy person with normal absorption, the body burns through only about 0.1% of its B12 stores per day. At that rate, a fully stocked liver could theoretically sustain you for over six years without any new B12 coming in. For someone with pernicious anemia who can’t absorb or recycle B12 at all, the turnover rate roughly doubles to 0.2% per day, which still translates to years of reserves if the pool was fully replenished. Health problems are estimated to begin once stores drop below about 300 micrograms, a fraction of the full reserve.

This is why B12 deficiency develops slowly. It can take several years of poor intake or absorption before symptoms appear. And it’s why a series of loading injections followed by monthly maintenance can keep stores reliably topped off.

Straightforward dietary deficiency usually begins with weekly injections for four weeks, then monthly.

The monthly maintenance schedule works for most people because a 1,000-microgram injection deposits enough B12 into your liver to cover daily needs for well beyond 30 days. For people with pernicious anemia or other absorption problems, injections every one to three months generally prevent relapse. Some people find their symptoms start returning before the next monthly shot is due, in which case their provider may shorten the interval.

Factors That Change How Long It Lasts

Several things can speed up how quickly your body depletes its B12 stores between injections:

  • Age: Atrophic gastritis, a condition that reduces stomach acid and the protein needed to absorb B12, affects about 2% of the general population but 8% to 9% of adults over 65. This means older adults lose B12 faster if they rely on oral sources, though injections bypass this problem.
  • Pernicious anemia: This autoimmune condition destroys the stomach cells that produce intrinsic factor, the protein required to absorb B12. People with pernicious anemia can’t absorb B12 from food or supplements at all and can’t recycle the B12 their body secretes into bile. Their stores deplete roughly twice as fast.
