High altitude has a similar effect, since lower oxygen levels already push blood vessels toward dilation before you even start moving.

A headache lasting under 48 hours that started during or right after exercise, feels like a dull throb, and fades on its own is the textbook benign pattern. But certain features should prompt a medical evaluation:

  • Sudden, severe onset. A headache that hits peak intensity within seconds, sometimes called a “thunderclap” headache, can signal bleeding around the brain. This is the most important red flag.
  • Duration beyond two days. Primary exertion headaches don’t last longer than 48 hours. If yours does, a secondary cause needs to be ruled out.
  • Confusion or excessive sleepiness. These suggest the brain is being affected beyond simple vascular stretching.

Easing into your workout over 10 to 15 minutes gives your vascular system time to adjust to increasing demand.

Hydration makes a meaningful difference, especially in warm conditions. Starting your workout well-hydrated and drinking throughout reduces the vascular instability that sets the stage for exertion headaches. If you’re exercising in heat or at altitude, you need more fluid than you think.

Breathing technique matters for strength training. Controlled exhaling during the effort phase of a lift prevents the extreme spikes in venous pressure that come from holding your breath against a closed airway. This won’t eliminate every headache, but it reduces the severity of the pressure surge.

For people who get frequent exertion headaches, a preventive medication taken 30 to 60 minutes before exercise can be effective. The standard option is an anti-inflammatory that works particularly well on exertional and pressure-related headaches. Starting at a low dose, you can adjust upward as needed.
