Each time the dose goes up, you may notice a brief return of side effects as your body adapts to the new level. This means the total adjustment period can stretch across several weeks if your target dose is 3 or 4 mg.

Drowsiness and fatigue are the side effects people notice most. They tend to peak within the first week at each new dose and improve significantly by weeks two to three. Some people find that drowsiness never fully disappears but becomes mild enough to ignore. Taking the dose at bedtime rather than in the morning can shift the sleepiest hours to when you’re already asleep.
Dry mouth often resolves within the first two weeks. If it persists beyond that point, it’s worth mentioning to your prescriber or dentist, since chronic dry mouth can affect dental health. Sugarless gum, ice chips, or saliva substitutes can help in the meantime.
Dizziness and low blood pressure are most noticeable when standing up quickly, especially in the first week or two. These symptoms typically ease as your cardiovascular system adjusts. Standing up slowly and staying hydrated can reduce the intensity while you wait it out.
Heart rate changes are real but usually modest. In one study of children and adolescents evaluated after an average of about four weeks on guanfacine, heart rate dropped by roughly 12 beats per minute compared to baseline. Blood pressure changes were small and not statistically significant. These cardiovascular shifts persist as long as you take the medication but return to normal after stopping.
Stomach pain, headache, and irritability show up in some people, particularly children. These tend to be milder and shorter-lived than drowsiness, often fading within the first two weeks at a stable dose.
Why the Extended-Release Version Matters
Guanfacine comes in two forms: immediate-release tablets (often used for blood pressure) and extended-release tablets sold as Intuniv (typically prescribed for ADHD). The extended-release version has a half-life of about 17 to 18 hours, meaning it releases slowly and stays in your system longer. This produces a smoother effect throughout the day, which generally means fewer sharp peaks of drowsiness compared to the immediate-release form. However, it also means side effects can linger longer after each dose, and the drug takes longer to fully clear your body when you stop.
What Happens When You Stop
Stopping guanfacine suddenly can cause a rebound effect, particularly a spike in blood pressure that typically shows up 2 to 4 days after the last dose. This happens because your nervous system, which had been running at a lower level, suddenly snaps back without the medication holding it in check. Symptoms can include headache, nervousness, rapid heartbeat, and elevated blood pressure.
To avoid this, the standard approach is tapering: reducing the dose by no more than 1 mg every 3 to 7 days. A gradual step-down gives your body time to readjust at each level. If you’re on 4 mg, for example, a full taper could take two to four weeks. Most rebound symptoms, if they occur at all with a proper taper, resolve within a week of reaching zero.
Factors That Affect Your Timeline
Higher doses generally produce more intense side effects and a longer adjustment period. Someone stabilizing at 1 mg may feel fully adjusted within a week or two, while someone titrating up to 4 mg may not feel settled for five or six weeks, since each dose increase restarts the adaptation clock.
Body size and metabolism play a role too. Children and smaller adults may be more sensitive to the sedating effects at any given dose. Taking guanfacine with a high-fat meal can increase how much of the drug your body absorbs, which can temporarily intensify side effects.
If you’re also taking a stimulant medication for ADHD, the combination can sometimes balance out certain side effects. Stimulants tend to increase alertness, which can offset guanfacine’s drowsiness. On the other hand, combining medications adds complexity, and everyone’s response is different.
When Side Effects Don’t Improve
If drowsiness, dizziness, or other side effects remain disruptive after three to four weeks at a stable dose, that’s a signal your current dose may be too high or the medication may not be the right fit. A dose reduction often brings relief quickly, usually within a few days, since the drug’s half-life means lower blood levels are reached within one to two days of a dose change. Some people find that a lower dose still provides the therapeutic benefit they need with side effects they can live with.
