Most common levofloxacin side effects, like nausea and diarrhea, clear up within a few days of finishing your course. But some effects, particularly those involving tendons, nerves, or the central nervous system, can persist for months or, in rare cases, become permanent. How long your side effects last depends largely on which type you’re experiencing.
Levofloxacin has a relatively short half-life of 6 to 8 hours, and roughly 87% of a dose leaves your body through urine within 48 hours. So the drug itself clears quickly. The side effects that linger do so because of changes the drug triggered in your tissues, not because it’s still circulating.
Common Digestive Side Effects
The most frequently reported side effects are nausea, vomiting, diarrhea, stomach pain, constipation, and heartburn. These typically resolve within a few days after your last dose, once the drug is no longer irritating your gut lining.
There’s one important exception: severe diarrhea caused by a bacterial infection called C. difficile. Levofloxacin can disrupt the balance of bacteria in your intestines enough to let this organism take over. This can show up as watery or bloody stools with cramping, and it may appear up to two months or more after you finish treatment. If diarrhea returns weeks later or becomes severe, that’s a sign something beyond a simple drug reaction is happening.
How Your Gut Recovers
Even after digestive symptoms stop, your gut bacteria need time to bounce back. Research from the Society for Healthcare Epidemiology of America found that bacterial counts and species diversity generally return to baseline within about 19 days for non-azithromycin antibiotics like levofloxacin. However, while most people’s microbiome looked similar in diversity by two months, the overall composition and antibiotic-resistance profile of their gut bacteria were still measurably different from their pre-antibiotic state. In practical terms, your digestion may feel normal relatively quickly, but the full microbial ecosystem takes longer to stabilize.
Tendon and Joint Problems
Levofloxacin belongs to the fluoroquinolone class of antibiotics, which carry an FDA boxed warning for tendon damage. This can range from tendon pain and swelling (tendinitis) to full tendon rupture, most commonly in the Achilles tendon. These problems can appear during treatment or surface weeks after you’ve stopped taking the drug.
Recovery from fluoroquinolone-related tendinitis varies widely. Mild cases may improve over several weeks with rest, while more severe tendon injuries can take months to heal. A full rupture typically requires surgical repair and a recovery period of several months or longer. People over 65, those taking corticosteroids, and anyone with kidney transplants face the highest risk. But the FDA notes these tendon problems can occur in patients of any age, even without pre-existing risk factors.
Nerve Damage and Neuropathy
Peripheral neuropathy, which shows up as tingling, burning, numbness, or pain in your hands and feet, is one of the more concerning potential effects. The FDA issued a specific safety communication warning that this nerve damage can start at any point during treatment and may last months to years after stopping the drug, or become permanent.
An FDA review of adverse event reports between 2003 and 2012 found a consistent association between fluoroquinolone use and disabling peripheral neuropathy. In some reported cases, symptoms had persisted for over a year despite discontinuation. Unlike the digestive side effects, nerve symptoms don’t reliably improve once the drug clears your system, because the damage occurs at the nerve fiber level.
If you notice new tingling, numbness, or unusual pain in your extremities while taking levofloxacin or shortly after finishing it, stopping the drug early (with your prescriber’s guidance) may help limit the extent of nerve injury.
Psychiatric and Central Nervous System Effects
Levofloxacin can cause a range of mental health and neurological symptoms, including insomnia, anxiety, confusion, disorientation, depression, and in rare cases, suicidal thoughts. The UK’s Medicines and Healthcare products Regulatory Agency has noted that there isn’t enough data to pinpoint a typical duration or a specific risk window for these reactions. They can emerge during treatment or after it ends.
For most people, symptoms like insomnia or mild anxiety resolve within days to a couple of weeks once the drug is out of their system. But the advisory specifically notes that patients should watch for psychiatric symptoms “even if it has been some time since they stopped taking the medication,” which signals that delayed or prolonged reactions do occur.
Fluoroquinolone-Associated Disability
In rare cases, patients develop a cluster of persistent symptoms involving tendons, muscles, joints, nerves, and the central nervous system simultaneously. The FDA has proposed a term for this pattern: fluoroquinolone-associated disability (FQAD). A 2021 review published in NeuroSci noted that while common side effects tend to disappear shortly after treatment, these rarer effects can potentially last a lifetime.
Researchers believe part of the explanation involves the way fluoroquinolone molecules bind to minerals in the body, forming stable complexes that may remain in tissues for extended periods and continue causing cellular damage long after the last dose. This is still an area of active investigation, and FQAD is not yet a formally recognized diagnosis, but the FDA’s labeling now clearly states that fluoroquinolones are “associated with disabling and potentially permanent, serious side effects.”
Factors That Affect Recovery Time
Several factors influence how long side effects stick around. Kidney function is a major one: levofloxacin is primarily eliminated through the kidneys, and people with reduced kidney function clear the drug more slowly, which increases both exposure and the chance of prolonged effects. Patients with a creatinine clearance below 50 mL/min require dose adjustments for this reason.
Age matters significantly. People 65 and older face elevated risk for tendon problems, especially if they’re also taking corticosteroids. The combination of age-related tissue changes and the drug’s effects on collagen creates a compounding vulnerability. Higher doses and longer courses also correlate with greater risk, though the FDA notes that disabling reactions can happen even with short courses in otherwise healthy people.
