The half-life of oral melatonin is about 1.8 to 2.1 hours, meaning your body eliminates half the active dose every two hours or so. At 20mg, there’s simply a lot more melatonin to clear.
How the 20mg Timeline Breaks Down
After swallowing a 20mg dose, melatonin typically reaches its peak concentration in your blood within about 1 to 1.5 hours. At that point, your levels will be far above the natural nighttime range your body produces on its own (which peaks around 60 to 70 pg/mL). From there, levels drop by roughly half every two hours.
Here’s what that looks like in practice. If your blood concentration peaks around 90 minutes after you take the pill, it will drop to the equivalent of a 10mg dose by about hour 3.5, a 5mg dose by hour 5.5, and a 2.5mg dose by hour 7.5. Even at that point, your melatonin levels are still well above what your pineal gland produces naturally. The type of melatonin tablet you took matters. Immediate-release melatonin hits its peak faster (around 35 to 40 minutes) and has a shorter half-life of roughly one hour. It floods your system quickly, then drops off. At 20mg, even the immediate-release version will linger for many hours because the sheer quantity overwhelms the normal clearance rate, but the tail end of the effect will be shorter than with an extended-release formula.

It’s processed by the same liver enzyme, so drinking coffee in the evening can compete with melatonin for clearance, potentially keeping melatonin levels higher for longer. Research has also shown that caffeine decreases the excretion of melatonin’s main breakdown product overnight, suggesting it genuinely slows how fast your body processes the hormone. Age plays a role too: older adults tend to metabolize melatonin more slowly, and people with liver conditions will clear it at a reduced rate. Smoking, on the other hand, speeds up the CYP1A2 enzyme and can shorten melatonin’s duration.
Next-Day Drowsiness at This Dose
The most common side effect of a 20mg dose is next-day drowsiness, sometimes called a “melatonin hangover.” Because your body needs 9 to 10 hours (or more) to fully clear this amount, taking it late at night can easily leave you groggy into the next morning. The Mayo Clinic advises against driving or operating machinery within five hours of taking melatonin at any dose. At 20mg, that window realistically extends further.
Other common effects that may linger into the next day include headache, dizziness, and mild nausea. These typically resolve as melatonin levels drop back to baseline, but if you took the dose less than 8 hours before you need to be alert, you’ll likely feel some residual sedation. The grogginess doesn’t always feel like sleepiness. Some people describe it more as mental fog or sluggishness that lifts gradually over the first few hours after waking.
Why 20mg Is Considered a High Dose
For context, most sleep-related uses of melatonin involve doses between 0.5 and 5mg. Doses above 5mg don’t appear to improve sleep onset or sleep quality in a dose-dependent way for most people. Your body’s natural nightly melatonin production creates blood levels roughly equivalent to taking 0.1 to 0.3mg as a supplement. A 20mg dose pushes blood concentrations far beyond anything your body would produce on its own.
Doses at or above 20mg have been used in some clinical research settings, particularly in oncology and neurology, but the general recommendation for sleep support stays at 5mg or less. At 20mg, you’re not getting proportionally better sleep. You are, however, getting a much longer duration of action and a higher chance of next-day side effects.
