A 2mg dose of Abilify (aripiprazole) takes roughly 16 to 20 days to fully clear your system. That timeline surprises most people because the dose is small, but aripiprazole is an unusually long-lasting medication. Its elimination half-life is about 75 hours (just over 3 days), meaning it takes that long for your body to remove just half the drug from your bloodstream.
Why Such a Small Dose Lasts So Long
The 2mg dose doesn’t clear faster than higher doses. Aripiprazole follows what pharmacologists call “linear kinetics” across its dose range, which means the rate your body breaks it down stays the same whether you took 2mg or 30mg. The difference is simply how much drug is in your system to start with, not how quickly each molecule gets processed. So while the total amount of aripiprazole circulating after a 2mg tablet is far less than after a 15mg tablet, the percentage removed per hour is identical.
The drug also distributes extensively throughout body tissues, not just in your blood. Its volume of distribution is about 404 liters, which is roughly 70 times the volume of your blood plasma. This means aripiprazole gets absorbed deep into tissues and then slowly releases back into the bloodstream to be eliminated, which is a major reason the half-life is so long.
The Full Elimination Timeline
Pharmacologists use “five half-lives” as the standard benchmark for when a drug is essentially gone from your body (over 96% eliminated). For aripiprazole, that math works out like this:
- 1 half-life (75 hours / ~3 days): 50% remains
- 2 half-lives (~6 days): 25% remains
- 3 half-lives (~9 days): 12.5% remains
- 4 half-lives (~12.5 days): 6.25% remains
- 5 half-lives (~15.5 days): about 3% remains
But aripiprazole also produces an active metabolite (a breakdown product that still has pharmacological effects) with an even longer half-life of about 94 hours. Five half-lives of this metabolite comes out to roughly 470 hours, or about 19.5 days. So while the parent drug is essentially gone in about two weeks, its active byproduct lingers a few days longer. For practical purposes, you can expect a single 2mg dose to be fully cleared within about three weeks.
Single Dose vs. Steady State
The timeline above applies to a single 2mg dose. If you’ve been taking 2mg daily for two weeks or more, the picture changes. Aripiprazole reaches “steady state” in about 14 days of consistent dosing. At that point, each new dose adds to drug that hasn’t fully cleared from previous doses, so total levels are higher than what a single tablet alone would produce. After stopping a daily regimen, clearance still follows the same half-life rules, but it starts from a higher baseline and can take slightly longer to reach zero.
Genetic Differences in Clearance Speed
Your body breaks down aripiprazole primarily through a liver enzyme called CYP2D6. About 5 to 10% of people of European descent (and smaller percentages in other populations) are “poor metabolizers,” meaning their version of this enzyme works much more slowly. For these individuals, the half-life of aripiprazole nearly doubles to about 146 hours, or roughly 6 days. Five half-lives at that rate means the drug could take over 30 days to fully clear.
Most people don’t know their CYP2D6 status. If you’ve noticed that medications in general seem to hit you harder or last longer than expected, slow metabolism could be a factor. Genetic testing is available, though it isn’t routinely ordered.
Other Medications Can Slow Clearance
Certain drugs compete for or block the same liver enzymes that process aripiprazole, effectively slowing its breakdown. One study found that quinidine (a CYP2D6 inhibitor) more than doubled aripiprazole blood levels. Fluoxetine (Prozac) and paroxetine (Paxil) are common antidepressants that have a similar effect. On a different pathway, the antifungal ketoconazole increased aripiprazole exposure by 63%.
If you take any of these medications alongside Abilify, the drug will stay in your system longer than the standard timeline suggests. The prescribing information calls for dose reductions when these combinations are used.
Drug Testing Considerations
Aripiprazole is not a substance that standard workplace drug panels screen for. Routine urine tests look for drugs of abuse like amphetamines, opioids, benzodiazepines, cannabis, and cocaine. Aripiprazole is an atypical antipsychotic and falls outside these categories. It would only show up on a specialized test ordered specifically to detect it, which is uncommon outside of forensic or clinical monitoring situations.
That said, aripiprazole and its metabolite are detectable in both blood and urine for the full duration of the elimination window described above. Given the long half-life, a blood test could pick up trace amounts for two to three weeks after a single 2mg dose, and potentially longer in poor metabolizers or people taking enzyme-inhibiting medications.
Why 2mg Feels Different Than Higher Doses
Although 2mg clears at the same rate as larger doses, the absolute amount of drug in your body is substantially less. Aripiprazole is prescribed across a wide range (2mg to 30mg), and the 2mg dose sits at the very bottom. At this level, side effects are generally milder and any lingering effects after stopping tend to be subtle. The drug’s pharmacokinetics are dose-proportional, so blood concentrations after 2mg are roughly one-seventh of what you’d see at 15mg (a common therapeutic dose for conditions like schizophrenia). The clearance timeline is the same, but what you actually feel during that window is proportional to how much you took.
