A single 50mg dose of Seroquel (quetiapine) is mostly eliminated from your body within about 35 hours. That estimate comes from the drug’s average half-life of roughly 7 hours, meaning your body clears half the drug every 7 hours. After five half-lives, the amount remaining is negligible, putting total clearance at around a day and a half for most people.
How the 7-Hour Half-Life Works
Every 7 hours, your liver breaks down about half of the quetiapine in your bloodstream. Starting from a 50mg dose, here’s roughly what that looks like:
- 7 hours: ~25mg remaining
- 14 hours: ~12.5mg remaining
- 21 hours: ~6.25mg remaining
- 28 hours: ~3mg remaining
- 35 hours: less than 2mg remaining
By 35 hours, the amount left is so small it’s unlikely to produce noticeable effects. At a low dose like 50mg, functional clearance may feel even faster because blood levels drop below the threshold that affects your brain receptors well before the drug is technically gone.
The Active Metabolite Lasts Longer
Your liver doesn’t just destroy quetiapine. It converts some of it into a breakdown product called norquetiapine, which is pharmacologically active. Norquetiapine has its own half-life of about 12 hours, nearly double that of quetiapine itself. This metabolite binds to many of the same brain receptors, including serotonin and dopamine receptors, and also has antidepressant-like properties.
Because of norquetiapine’s longer half-life, traces of the drug’s active effects can persist beyond the 35-hour window for the parent drug. Full clearance of the metabolite takes closer to 60 hours (five cycles of 12 hours). PET imaging studies confirm that receptor activity from quetiapine and its metabolite can be maintained for up to 12 hours after a single dose. For a 50mg dose, the metabolite levels are relatively low to begin with, so this extended tail is subtle, but it’s worth knowing about if you’re tracking how you feel in the days after your last dose.
Immediate-Release vs. Extended-Release
Seroquel comes in two formulations: immediate-release (IR) and extended-release (XR). The elimination half-life is the same for both, approximately 7 hours. The difference is in how quickly the drug reaches peak levels in your blood. Immediate-release tablets hit a higher peak faster and then drop off, while extended-release tablets produce a flatter, more gradual curve. The total amount of drug your body absorbs is essentially equivalent between the two.
What this means practically: if you take 50mg of the XR version, it may take longer to feel the effects wearing off because the drug enters your system more slowly. But the total time until the drug is fully cleared stays roughly the same.
Factors That Slow Clearance
The 7-hour half-life is an average. Several things can make your body process quetiapine significantly faster or slower.
Liver Function
Quetiapine is almost entirely broken down by the liver, specifically by an enzyme system called CYP3A4. If your liver isn’t working at full capacity due to disease, aging, or other conditions, clearance slows and the drug stays in your system longer. The FDA label notes that steady-state concentrations are reached within about two days of regular dosing, which gives you a sense of how quickly the body normally processes and rebalances levels.
Other Medications
This is probably the biggest variable. Drugs that inhibit the CYP3A4 enzyme dramatically slow quetiapine clearance. In clinical studies, the antifungal ketoconazole (a strong CYP3A4 inhibitor) reduced quetiapine clearance by 84% and more than doubled the time the drug stayed active in the blood. On the other end, carbamazepine (an anti-seizure medication that revs up CYP3A4 activity) increased clearance 7.5-fold, meaning the drug was eliminated far faster than normal. If you take other medications, they could meaningfully shift your clearance timeline in either direction.
Age
Older adults tend to clear quetiapine more slowly. Liver enzyme activity naturally declines with age, which extends the effective half-life. If you’re over 65, the drug may linger in your system somewhat longer than the standard 35-hour estimate.
Single Dose vs. Regular Use
If you’ve been taking 50mg regularly rather than as a one-time dose, the math changes. With consistent dosing, quetiapine accumulates to what’s called steady state, where the amount entering your system each day equals the amount leaving. The FDA notes that steady state is typically reached within about two days of regular dosing.
Once you stop after regular use, it still takes about five half-lives from your last dose for the drug to fully clear, so the timeline is similar. But because your starting blood levels at steady state are higher than after a single dose, you may notice the effects tapering more gradually. Norquetiapine levels also build up with regular dosing, adding to this extended tail. For a low dose like 50mg, the overall clearance window after stopping is still roughly two to three days for most people.
Drug Testing Considerations
Standard drug panels (the kind used for employment screening) do not test for quetiapine. It’s not a controlled substance and won’t show up as a positive for any common drug class. However, there have been rare reports of quetiapine causing false positives for certain substances on immunoassay screening tests, particularly for methadone or tricyclic antidepressants. A confirmatory test will always rule these out. If you’re concerned about a specific test, the drug and its metabolites are generally undetectable in blood within two to three days of a single 50mg dose.
