A broken clavicle takes eight to 12 weeks to heal in most adults. Children heal faster, often in six to eight weeks, while older adolescents may need closer to 10 to 12 weeks. The actual timeline depends on the severity of the break, whether surgery is needed, and several lifestyle factors that can speed up or slow down bone repair.
Healing Timeline by Age
Adults can expect their collarbone to reach bony union somewhere between eight and 12 weeks after the fracture. Most people start feeling significantly better well before that, but the bone itself is still knitting together and remains vulnerable to re-injury during that window.
Children’s bones heal considerably faster. In younger kids, a clavicle fracture can heal in as little as six weeks. Older children and adolescents fall closer to the adult range, typically needing 10 to 12 weeks. This difference comes down to how active bone growth is at younger ages. The closer a child is to skeletal maturity, the more their healing timeline resembles an adult’s.
What Recovery Looks Like Week by Week
The first two weeks are the most uncomfortable. Pain and swelling peak early, and your arm will be in a sling full-time. Even during this initial phase, gentle pendulum exercises (letting your arm hang and swing in small circles) help prevent your shoulder from stiffening. After the first week, you can begin assisted movements and slowly work toward raising your arm past shoulder level with help.
Between weeks two and six, you enter the early motion phase. Passive and assisted range-of-motion exercises expand gradually. By around week four, gentle isometric exercises (tensing muscles without moving the joint) become safe. By week five, light resistance band work can begin. During this entire stretch, lifting is limited to one to two pounds with the affected arm.
From weeks six to 12, the bone is solidifying and you can start increasing what you carry, up to about 10 pounds. Strengthening exercises using light weights under five pounds become part of your routine. Most people come out of the sling around the six-week mark, and this is also when driving typically becomes an option again, as long as you have enough shoulder control to steer safely.
Full return to sports or heavy physical activity generally falls in the four-to-six-month range. Even after the bone has healed on X-ray, the surrounding muscles need time to rebuild strength and coordination.
Surgery vs. Nonsurgical Healing
Most clavicle fractures heal without surgery. A sling, pain management, and gradual rehabilitation are enough. But displaced fractures (where the bone ends have shifted apart) or breaks with multiple fragments sometimes require surgical fixation with a plate and screws.
Surgery shortens the time to bony union by a few weeks. One study found a mean union time of about 12 weeks with surgery compared to 14 weeks without. Another showed a larger gap: roughly 16 weeks after surgery versus 23 weeks with conservative treatment. Union rates are also higher with surgical repair, ranging from 93 to 100 percent compared to 82 to 94 percent for nonsurgical management.
That said, surgery introduces its own recovery demands. You still wear a sling, still do physical therapy, and still face weeks of restricted activity. The rehabilitation protocol after surgical fixation follows a similar week-by-week progression as nonsurgical recovery. The main advantage is a more predictable healing timeline, especially for fractures that carry a higher risk of not healing on their own.
Factors That Slow Healing
Smoking is the single biggest modifiable risk factor. Smokers have 2.2 times the risk of delayed union or nonunion (the bone failing to heal at all) compared to nonsmokers. That elevated risk holds regardless of whether the fracture is treated with surgery or a sling. If you smoke and break your collarbone, quitting or at least stopping during recovery meaningfully improves your odds.
Other factors that can slow bone repair include diabetes, poor nutrition (particularly low calcium and vitamin D intake), and older age. The location of the fracture matters too. Midshaft fractures, which account for the majority of clavicle breaks, generally heal more reliably than fractures near the outer end of the bone closest to the shoulder.
What You Can and Can’t Do During Recovery
Driving is off the table while you’re in a sling. Most people wear a sling for about six weeks, and that’s typically when driving can resume, provided you can grip the wheel and turn it without pain. Your doctor may clear you earlier or later depending on which side is injured and how you’re progressing.
Lifting restrictions are strict in the early weeks and loosen gradually. For the first six weeks, keep the affected arm to one or two pounds at most. Think: a phone, a cup of coffee, a book. From six to 12 weeks, you can work up to 10 pounds. Heavier lifting, pushing, and pulling should wait until you’ve been cleared, usually after three months at the earliest.
Sleeping is one of the most underestimated challenges. Lying flat puts pressure on the collarbone, so many people sleep semi-upright in a recliner or propped on pillows for the first few weeks. Side sleeping on the uninjured side becomes possible sooner, but rolling onto the broken side can wake you with a sharp reminder that the bone isn’t ready yet.
Signs the Bone Isn’t Healing Normally
Most clavicle fractures heal without complications, but nonunion does happen. If your pain plateaus instead of gradually improving after the first month, or if you still have significant pain and instability at the fracture site beyond 12 weeks, that’s worth a follow-up X-ray to check progress. A visible or palpable bump over the fracture site is normal and expected. That’s the bone callus forming, and it’s actually a sign healing is on track. The bump shrinks over time, though in thinner individuals it may remain slightly noticeable.
