A simple stress fracture in a metatarsal and a shattered heel bone are worlds apart in terms of what to expect, so the real answer depends on your specific injury.
How Bone Healing Actually Works
Your body repairs a broken bone in three overlapping stages. The first is an inflammatory phase that begins within hours. The area swells, bruises, and hurts as broken blood vessels form a clot. That clot becomes the foundation for new bone. This phase lasts a few days.
Next comes the repair phase, lasting days to weeks. The blood clot transforms into a soft callus made of cartilage and other tissue. This callus provides some stability but isn’t strong. Over several weeks it hardens into a firm callus that’s weaker than normal bone but solid enough to bear some use. Finally, a remodeling phase stretches over months to years as the hard callus gradually converts into mature bone that matches the original structure. This is why your foot can feel “healed” long before it’s truly back to full strength.
Timelines by Fracture Type
Metatarsal Fractures
The metatarsals, the five long bones in the middle of your foot, are the most commonly broken foot bones. A straightforward metatarsal fracture typically heals in about six weeks. You’ll transition out of a walking boot after two to six weeks and gradually increase activity as pain allows. Residual symptoms like stiffness, mild swelling, and decreased strength can linger for three to six months even after the bone itself has knit together.
Jones Fractures
A Jones fracture is a specific break at the base of the fifth metatarsal (the bone along the outer edge of your foot). It deserves its own category because the fracture site has limited blood supply, which slows healing considerably. Recovery takes several months rather than weeks, and roughly one-third of Jones fractures fail to heal on their own, a condition called nonunion. Whether you’re treated with a cast or surgery, expect to keep weight off the foot for six to eight weeks before starting rehabilitation.
Stress Fractures
Stress fractures are tiny cracks caused by repetitive force rather than a single injury. They’re common in runners and athletes. Most foot stress fractures heal in six to eight weeks with rest. You’ll need to avoid sports and high-impact activities for a few months, even after the bone has healed, to prevent re-injury.
Heel (Calcaneus) Fractures
The heel bone is the longest recovery in the foot. A calcaneal stress fracture may heal within a few months, but a severely broken heel, especially one that damages the joint surface, can take months or even years to fully recover from. Some people need multiple surgeries. These injuries can cause permanent changes including arthritis in the joint below the ankle, chronic pain, and foot deformity if the bones heal in the wrong position.
What Recovery Looks Like Week by Week
For a typical foot fracture, the first one to three weeks are the acute phase. You’ll be in a cast or walking boot, likely using crutches, and keeping weight off the foot. The focus is on controlling swelling and protecting the break.
Around weeks four to six, you enter a subacute phase. If healing is on track, you’ll start gentle strengthening exercises and begin putting more weight through the foot. This is when most people transition out of a boot for simpler fractures.
Weeks six through sixteen are the advanced strengthening phase. You’ll work on balance, single-leg stability, and rebuilding the small muscles in your foot. This phase determines how well you return to normal activity. Skipping it or rushing through it is a common reason people experience lingering pain months later. For athletes, returning to sport-specific training typically falls at the tail end of this window or beyond.
Factors That Slow Healing
Smoking is one of the biggest controllable risk factors. Smokers have roughly 2.5 times the rate of nonunion compared to nonsmokers, according to a large meta-analysis published in The Lancet. Quitting at least four weeks before any surgical procedure significantly reduces infection risk as well.
Diabetes impairs healing because elevated blood sugar damages small blood vessels and reduces blood flow to bone. Poor nutrition, particularly low calcium and vitamin D intake, also slows the process. Age plays a role too: children’s fractures often heal in half the time of an adult’s, while older adults may take longer than average.
The location of the fracture matters as much as anything. Bones with rich blood supply heal faster. The Jones fracture site and the heel bone are both relatively blood-poor areas, which is why they’re notorious for slow or failed healing.
What “Fully Healed” Really Means
There’s a gap between the bone being structurally repaired and your foot feeling normal again. The bone itself may knit in six to eight weeks, but stiffness, swelling after long days on your feet, and reduced strength commonly persist for three to six months. For severe fractures like displaced heel injuries, some degree of stiffness or discomfort can be permanent.
Your return to full activity depends on more than just the bone. The muscles, tendons, and ligaments around the fracture weaken during immobilization. Rebuilding that support system through progressive exercise is what ultimately gets you back to walking, running, or standing comfortably for extended periods.
