Most pulled muscles heal within a few weeks, but the actual timeline depends on how badly the muscle is damaged. A minor strain can feel better in one to three weeks, while a moderate tear may take several weeks to months, and a complete rupture requiring surgery can sideline you for four to six months.
Severity Determines Your Timeline
A “pulled muscle” is the common term for a muscle strain, and strains fall into three grades based on how much of the muscle fiber is damaged.
Grade 1 (mild): The muscle is stretched and slightly damaged but not torn through. This is the most common type. You’ll feel tightness, mild pain, and maybe some tenderness, but you can still move the area. These typically heal within a few weeks.
Grade 2 (moderate): Some or even most of the muscle fibers are torn. You’ll notice a clear loss of strength and limited range of motion, along with swelling and more significant pain. Recovery takes several weeks to months.
Grade 3 (severe): The muscle is torn all the way through. This is a complete rupture and often requires surgery. After surgical repair, healing takes four to six months. You may feel a pop at the time of injury, see significant bruising, or even notice a visible gap or dent in the muscle.
What Happens Inside the Muscle as It Heals
Your body repairs a pulled muscle in overlapping stages, and understanding these helps explain why rushing back too soon causes setbacks.
In the first 6 to 24 hours, the injured area bleeds internally and begins to swell. This transitions into an inflammatory phase that peaks around days one to three. Inflammation gets a bad reputation, but it’s actually your body’s cleanup crew, clearing damaged tissue and sending in the cells needed for repair. They can interfere with this natural healing process, potentially slowing long-term recovery.

Finally, a remodeling phase begins (typically around one to six weeks in) where that new tissue gradually strengthens and reorganizes to handle normal loads again. Location Matters Too
Not all muscles heal at the same pace. Large, high-tension muscles like your hamstrings are notorious for longer recoveries. Even a mild hamstring strain can take less than a week or stretch to several months depending on the grade. Muscles that cross two joints, like the hamstrings (which cross both the hip and knee) and the calf (crossing the knee and ankle), tend to be under more mechanical stress during everyday movement, which can slow recovery.
Muscles with a good blood supply generally heal faster than those in areas with less circulation. Your quadriceps and calves, for example, get robust blood flow and tend to recover more predictably than deeper muscles with less vascular access.
How to Manage the First Few Days
The traditional advice of rest and ice has been updated. A framework published in the British Journal of Sports Medicine recommends a two-phase approach called PEACE and LOVE, which better reflects what injured tissue actually needs.
In the first one to three days, focus on protection: unload the muscle and limit movement to prevent further damage. Elevate the injured area above your heart to help fluid drain. Use compression with a bandage or tape to limit swelling. And here’s the part that surprises many people: skip the ice and anti-inflammatory painkillers if you can manage the discomfort. There is no high-quality evidence that icing improves outcomes for soft-tissue injuries, and anti-inflammatories may actually compromise tissue healing, especially at higher doses.
After those initial days, the approach shifts. Start adding gentle, pain-free movement and light cardiovascular activity. This increases blood flow to the injured area and supports healing. The key word is pain-free. If an activity hurts, you’re loading the tissue too much, too soon.
Why Gradual Loading Prevents Setbacks
One of the biggest mistakes people make with a pulled muscle is going from rest straight back to full activity. Research on rehabilitation shows that the muscle needs a gradual exposure phase where it’s introduced to increasing loads over time. Without this ramp-up, the healing tissue gets overwhelmed, leading to more damage and a longer overall recovery.
This is where eccentric exercises become important. These are movements where the muscle lengthens under tension, like slowly lowering a weight or walking downhill. They’re a cornerstone of muscle strain rehab, but they need to start at low intensity. If you skip the adaptation period and jump into heavy eccentric work, you’ll likely aggravate the injury and abandon the program altogether. Starting with lighter versions allows the muscle to build tolerance before progressing.

In a study tracking hamstring injuries over two decades, 18% of all hamstring strains were recurrences. Of those, 69% happened within just two months of the original injury. That’s a clear signal that the first eight weeks after a strain are a high-risk window.
When You’re Ready to Return to Full Activity
Feeling less pain is not the same as being healed. The benchmarks used in sports medicine to clear someone for running and jumping give a useful framework for anyone recovering from a pulled muscle. You should have full, pain-free range of motion in the joint that matches your uninjured side. The injured muscle should be at least 80% as strong as the same muscle on your other limb. And you should be able to perform functional movements, like walking with a normal gait and doing controlled single-leg exercises, with good mechanics and no pain.
If you can’t do those things comfortably, the muscle isn’t ready for high-impact activity, regardless of how many weeks have passed.
Signs a Strain Needs Medical Attention
Most Grade 1 strains heal fine on their own with basic self-care. But certain signs point to a more serious injury that needs professional evaluation. If you heard or felt a pop at the time of injury, can’t bear weight or use the muscle at all, notice a visible dent or gap in the muscle, or see extensive bruising that spreads well beyond the injury site, you may have a Grade 2 or Grade 3 tear. A complete muscle rupture won’t heal on its own and typically requires surgical repair followed by a structured rehabilitation program lasting several months.
