How Long Does a Sprained ACL Take to Heal?

A sprained ACL takes anywhere from a few weeks to over a year to heal, depending on severity. A mild sprain with the ligament still intact can recover in 4 to 12 weeks with rest and rehabilitation, while a complete tear treated with surgery typically requires 9 to 12 months before a full return to activity.

Why ACL Injuries Heal Slowly

The ACL sits deep inside your knee joint, bathed in synovial fluid, and has a limited blood supply compared to ligaments on the outside of the joint. Blood flow is what drives tissue repair. When researchers compared the ACL to the MCL (the ligament on the inner side of the knee), they found the MCL responded to injury with a large increase in blood flow and robust new blood vessel growth, while the ACL showed only a modest increase in blood vessel volume, no meaningful increase in blood flow, and actually began to shrink after injury. This poor vascular response is the main reason ACL tears rarely heal on their own and often require surgical intervention.

When healing does occur in a ligament, the body moves through three overlapping phases. The inflammatory phase peaks in the first five days as immune cells flood the area. A proliferative phase follows over the next one to two weeks, laying down new tissue. Then a remodeling phase begins around days 14 to 21, where the body reorganizes that new tissue into something functional. For most ligaments, remodeling continues for months. For the ACL specifically, the process is often incomplete without surgical help.

Healing Time by Injury Severity

ACL sprains are graded on a three-tier scale. The grade determines not just how long recovery takes but whether surgery is likely.

  • Grade 1 (mild sprain): The ligament is stretched but still intact. You’ll have some pain and swelling, but the knee remains stable. Most people recover in 4 to 6 weeks with physical therapy, though full confidence in the knee during cutting or pivoting sports may take closer to 8 to 12 weeks.
  • Grade 2 (partial tear): The ligament is partially torn, leaving the knee somewhat unstable. Recovery without surgery can take 3 to 6 months, though some partial tears eventually require reconstruction if the knee doesn’t feel stable enough for your activity level.
  • Grade 3 (complete tear): The ligament is fully ruptured. This is the most common scenario people face when they hear “ACL tear.” Recovery with surgical reconstruction takes 6 to 12 months. Without surgery, some people adapt over several months using strengthening alone, but this path works best for those who don’t need to return to pivoting sports.

What Recovery Looks Like After Surgery

ACL reconstruction replaces the torn ligament with a graft, either from your own tissue (patellar tendon or hamstring) or from a donor. The graft needs time to incorporate into the bone and mature into functional tissue, which is why the rehab timeline stretches so long even when the surgery itself goes well.

Rehabilitation follows a structured progression through four phases. In the first six weeks, the focus is protecting the graft while restoring range of motion and getting the quadriceps firing again. Knee bending should reach within 10 degrees of the opposite leg during this window, and straightening the knee fully is a priority from the start.

From weeks 7 through 14, the focus shifts to building muscular endurance with light loads and higher repetitions. Swelling typically resolves by about week 13. By weeks 15 through 21, you’re progressing to heavier strengthening, and a running and agility program begins. This is when the knee starts to feel more like a knee again rather than a rehab project.

After about 5 to 6 months, training shifts toward power, speed, and sport-specific movements. Most people notice the biggest psychological shift during this period, as the knee begins handling forces similar to what it’ll face in competition. Research shows that athletes who score above 60% on psychological readiness questionnaires at the 6-month mark are highly likely to return to their sport by 12 months.

When You’re Actually Cleared to Return

The 9-month mark is treated as the earliest safe window for returning to sport after ACL reconstruction, and there’s strong data behind that number. One large cohort study found a 51% reduction in re-injury risk for every month return to sport was delayed, up to 9 months. Athletes who returned before 9 months had a 7 times higher risk of a new ACL injury.

Clearance isn’t based on time alone. You’ll need to pass a series of objective benchmarks. The standard criteria include quadriceps strength at least 90% of your uninjured leg, scoring 90% or better on a battery of four hop tests, no swelling, full range of motion, and a thigh circumference difference of less than 1.5 centimeters between legs. Most protocols require passing at least 6 out of 7 checklist items before you’re cleared for unrestricted activity.

For patients who received a donor graft rather than their own tissue, the return-to-competition window extends to 12 months or longer, since donor grafts tend to incorporate more slowly.

Recovery Without Surgery

Not everyone with a torn ACL needs reconstruction. People who are older, less active, or willing to modify their activities can often manage with physical therapy alone. The rehab focus is the same: restoring range of motion, building quadriceps and hamstring strength, and improving neuromuscular control so the surrounding muscles compensate for the missing ligament.

Spontaneous healing of a complete ACL tear is rare but not impossible. In one case series of 21 patients managed without surgery, 18 of 19 who completed follow-up MRI at one year showed a healed ACL with normal-looking fibers and trajectory. This is the exception rather than the rule, and it’s not something to count on, but it does happen. The non-surgical path typically requires 3 to 6 months of dedicated rehabilitation before the knee feels reliable for daily activities. Pivoting and cutting sports remain risky without a functioning ACL.

Re-Injury Risk After Recovery

Even after full recovery and surgical reconstruction, the risk of a second ACL injury is real. About 1 in 17 patients (6%) sustain another ACL injury within two years of returning to sport. Half of those are graft failures on the same knee, and the other half occur on the opposite knee. Overall, people returning from ACL reconstruction are five times more likely to suffer a second ACL injury in the first 24 months compared to someone who has never injured their ACL.

This elevated risk is why the return-to-sport criteria are so strict, and why rushing the timeline, even when the knee feels good, consistently leads to worse outcomes. The graft may feel strong at 6 months, but the biological maturation process is still underway, and the neuromuscular patterns that protect the knee during unpredictable movements take time to rebuild.
