How Long Does a Sprained Ankle Last: Recovery Times

Most sprained ankles heal within one to four weeks, though severe sprains can take two to three months. The exact timeline depends on how badly the ligament is damaged, where the sprain is located, and how consistently you rehabilitate the ankle during recovery.

Recovery Time by Severity

Ankle sprains are graded by how much damage the ligament sustains. A mild sprain (grade 1) involves stretching or tiny tears in the ligament and typically heals in one to two weeks. A moderate sprain (grade 2) means the ligament is partially torn, and recovery takes two to four weeks. A severe sprain (grade 3) is a complete ligament tear, requiring six to eight weeks to heal. Some complete tears need surgery, which can push recovery to several months.

These timelines reflect when most people can return to normal daily activities, not when the ligament is fully restored. Ligament tissue continues remodeling for months after you feel better, which is why re-injury rates are highest in the weeks after you think you’ve healed.

High Ankle Sprains Take Longer

The most common ankle sprain happens on the outside of the ankle when you roll your foot inward. A high ankle sprain is a different injury altogether. It damages the ligaments above the ankle joint that connect the two bones of the lower leg. These sprains are less common but significantly slower to heal, with an average recovery time of 8 to 12 weeks. They’re more frequent in sports that involve cutting, pivoting, or direct contact to the lower leg.

If your pain is centered above the ankle bone rather than below it, or if squeezing the two shin bones together reproduces the pain, you may be dealing with a high ankle sprain rather than a standard one.

What Happens Inside the Ankle During Healing

Ligaments heal in three overlapping phases, and understanding them helps explain why recovery timelines aren’t always straightforward.

The first phase is inflammation, which begins within minutes of the injury and lasts about 48 to 72 hours. This is the period of peak swelling, heat, and pain. Your body floods the area with immune cells to clear damaged tissue. The second phase is repair, lasting several weeks, during which specialized cells lay down new collagen to rebuild the ligament. The third phase, remodeling, is the longest. It begins a few weeks after injury and continues for months to even years as the new collagen fibers gradually reorganize and strengthen.

This is why an ankle can feel fine for walking but still give out during a quick lateral movement. The new tissue needs time under controlled stress to regain the tensile strength of healthy ligament.

Rehabilitation and Getting Back to Activity

There’s an ongoing debate about whether early movement or immobilization leads to faster recovery. A large meta-analysis comparing the two approaches found no significant differences in pain, functional improvement, re-injury rates, or return to daily activities. In practical terms, this means you don’t need to panic about doing it “wrong.” Both approaches, whether using a brace with gentle early movement or a short period of immobilization, lead to similar outcomes for most people.

What does matter is progressive rehabilitation. Once the acute pain and swelling subside, rebuilding strength, balance, and coordination in the ankle is essential. For returning to sports or high-impact activities, clinicians look at several benchmarks: pain-free participation in sport-specific movements, restored range of motion and muscle strength, adequate balance on the injured leg, and the ability to hop, jump, and change direction without hesitation or discomfort. There aren’t universally agreed-upon numerical cutoffs for these tests, so the process is somewhat individualized.

Why Some Sprains Don’t Fully Resolve

One of the most underappreciated facts about ankle sprains is how often they lead to lasting problems. Up to 40% of people with lateral ligament injuries continue to experience residual pain, swelling, or instability after their initial recovery window. Some studies put the rate of chronic ankle instability even higher, near 70%, particularly in people who return to activity before adequate rehabilitation.

Within the first 12 months after a sprain, about 16% of people experience a second sprain on the same ankle. That number climbs in athletes and in people who skip balance and strength training during recovery. The ankle’s proprioception, its ability to sense its own position in space, is disrupted by ligament damage. Without targeted exercises to retrain that sense, the ankle remains vulnerable to rolling again in the same way.

Signs Your Injury May Be More Serious

Not every twisted ankle is a sprain. Fractures can feel similar, and the distinction matters because a broken bone requires different management. Emergency physicians use a set of criteria to decide whether an X-ray is needed: if you have tenderness when pressing on the bony tips at either side of the ankle, tenderness at the base of the outer foot bone or the bone on the inner arch, or if you couldn’t take four steps either immediately after the injury or upon evaluation, imaging is warranted.

Persistent inability to bear weight, worsening swelling after the first few days, or bruising that spreads significantly are also signals that the injury may be more than a simple sprain. A sprain that doesn’t improve at all after two weeks of rest and basic care deserves a closer look.
