Alcohol detox typically lasts five to seven days for most people, with symptoms peaking between 24 and 72 hours after the last drink. The full timeline varies depending on how much and how long you’ve been drinking, your history of past withdrawals, and whether you have other medical conditions. A small number of people experience lingering symptoms that stretch on for weeks or even months.
What Happens in Your Brain During Detox
Alcohol enhances the activity of your brain’s main calming chemical while suppressing the main excitatory one. Drink heavily for long enough and your brain adapts, dialing down its own calming signals and ramping up excitatory ones to compensate. When you suddenly stop drinking, that compensation doesn’t reverse instantly. You’re left with too little calming activity and too much excitatory activity, which is why withdrawal feels like your nervous system is in overdrive: racing heart, tremors, anxiety, and in severe cases, seizures.
This chemical imbalance is measurable. Studies of people in acute withdrawal show significantly lower levels of the brain’s calming chemical and significantly higher levels of the excitatory one compared to people who aren’t withdrawing. The ratio between the two is what drives the severity of your symptoms and largely determines how long your body needs to recalibrate.

6 to 12 Hours
The first symptoms usually show up within six to 12 hours after your last drink. These tend to be mild: headache, anxiety, nausea, insomnia, and a general feeling of unease. Tremors, the classic “shakes,” often start within five to 10 hours. For people with mild withdrawal, this may be as bad as it gets.
12 to 24 Hours
Some people begin experiencing hallucinations in this window, typically visual or tactile (seeing or feeling things that aren’t there). These can last up to two days. Not everyone gets them, and they don’t necessarily mean you’re headed for the most dangerous stages of withdrawal, but they’re a signal that your withdrawal is more than mild.
24 to 72 Hours
This is the peak danger zone. For most people with mild to moderate withdrawal, symptoms hit their worst point somewhere in this range and then start improving. Tremors typically peak at 24 to 48 hours. Seizure risk is highest at 24 to 48 hours as well, and it’s common for multiple seizures to occur over the course of several hours.
Delirium tremens, the most severe form of withdrawal, usually appears between 48 and 72 hours after the last drink, though it can be delayed by more than a week. It involves severe confusion, rapid heartbeat, fever, and agitation. Its peak intensity typically hits four to five days after the last drink. Delirium tremens is a medical emergency.
Days 4 to 7
Most withdrawal symptoms improve within five days. By the end of the first week, the acute physical symptoms (shaking, sweating, nausea, elevated heart rate) have generally resolved. People with uncomplicated withdrawal who are being monitored in an outpatient setting are typically checked daily for up to five days, after which severe withdrawal is unlikely to develop if it hasn’t already.
What Makes Detox Longer or More Severe
Past withdrawal experience is usually the single best predictor of what your next withdrawal will look like. If you’ve previously had seizures, hallucinations, or delirium during withdrawal, you’re more likely to experience them again. Each successive withdrawal episode also tends to be worse than the last, a phenomenon sometimes called “kindling,” where the brain becomes increasingly sensitive to the effects of alcohol removal.
Higher daily consumption generally means more severe withdrawal, though people drinking at moderate-to-heavy levels (roughly eight to 10 standard drinks per day) can still develop serious complications. A telling sign of dependence is waking up with withdrawal symptoms like tremor, nausea, or anxiety that go away with a morning drink. If that pattern sounds familiar, your detox is more likely to be on the severe end.
Several other factors raise the risk:
- Other substance use. Regular use of sedatives, stimulants, or opioids alongside alcohol can intensify withdrawal. Concurrent dependence on both alcohol and sedatives is particularly dangerous.
- Medical conditions. Liver disease, epilepsy, infections, head injuries, or significant nutritional deficiencies make complications more likely.
- Psychiatric conditions. Pre-existing anxiety, depression, or psychosis can worsen during withdrawal and extend the overall recovery timeline.
People who drink heavily but only a few days per week generally don’t experience severe withdrawal, though other conditions like underlying anxiety or epilepsy may flare once the alcohol clears their system.
What Medical Detox Looks Like
Medical detox uses sedative medications to calm the same brain system that alcohol was artificially stimulating. These medications are given either on a fixed schedule or triggered by symptoms as they arise. The symptom-triggered approach tends to result in a shorter treatment duration, with a median of about nine hours of active medication compared to roughly 68 hours on a fixed schedule.
For people with mild withdrawal and a low risk of complications, a 36-hour observation period is often sufficient. If more severe withdrawal hasn’t developed by that point, it’s unlikely to. More severe cases require longer monitoring, sometimes in an inpatient setting, especially when there’s a risk of seizures or delirium tremens.
The goal of medical detox isn’t just comfort. Untreated severe withdrawal can be fatal. Medical supervision allows providers to intervene before symptoms escalate to dangerous levels.
Post-Acute Withdrawal: The Longer Tail
Even after the acute phase wraps up within a week, some people enter a prolonged withdrawal period known as post-acute withdrawal syndrome, or PAWS. Unlike the dramatic physical symptoms of the first few days, PAWS is subtler and more psychological. The most common symptoms include depression, irritability, mood swings, anxiety, sleep problems, difficulty concentrating, and cravings for alcohol.
PAWS can persist for months, and in some cases, more than a year. It’s one of the main reasons early sobriety feels so difficult even after the body has physically cleared the alcohol. The brain’s chemical balance doesn’t snap back to normal overnight. The calming and excitatory systems that were thrown off by chronic drinking need time to fully recalibrate, and during that process, your emotional resilience and cognitive sharpness may feel noticeably off.
Understanding that these symptoms are a normal part of the recovery timeline, not a sign of personal failure, can make a significant difference in staying on track. Sleep disturbances and mood instability in particular tend to be the last symptoms to resolve.
