But “staying in your system” depends on what you mean: whether you’re concerned about next-morning impairment, a drug test, or how long it takes for every trace to clear. Each of those timelines is different.

How Quickly Your Body Clears Ambien

Zolpidem is processed primarily by the liver. In healthy adults, the elimination half-life averages about 2.2 hours, with a typical range of 1.6 to 2.4 hours. The extended-release version (Ambien CR) has a similar terminal half-life of 2 to 3 hours, but it’s designed to release the drug in two phases. Standard workplace drug panels don’t typically screen for zolpidem. But specialized tests can detect it, and the windows vary by sample type.

  • Urine: In a study where volunteers took a single 10 mg dose, zolpidem was detectable in urine for up to 60 hours (about 2.5 days). Concentrations peaked around 12 hours after the dose and tapered from there. Specimens were collected every 12 hours for six days, and the drug was undetectable after the 60-hour mark.
  • Blood: Zolpidem clears from the bloodstream faster than from urine. Based on its half-life, blood levels drop below detectable thresholds within roughly 8 to 16 hours for most people, though this varies with individual metabolism.
  • Hair: A single dose of zolpidem can be detected in hair samples taken 3 to 5 weeks after exposure. Concentrations are extremely low (measured in trillionths of a gram per milligram of hair), but modern forensic techniques can pick them up. Hair testing is uncommon outside of forensic investigations.

Why Women Clear Ambien More Slowly

Women eliminate zolpidem from their bodies at a slower rate than men. This difference is significant enough that the FDA required separate dosing recommendations in 2013. In trials involving roughly 500 people who took a standard 10 mg dose, about 15% of women still had blood concentrations above 50 ng/mL eight hours later. Only 3% of men had levels that high at the same time point.

For the extended-release formulation, the gap narrows but persists: 33% of women versus 25% of men exceeded that threshold at eight hours. The FDA now recommends that women start at 5 mg for immediate-release Ambien and 6.25 mg for the extended-release version, essentially half the original standard dose. Men are also encouraged to consider these lower doses.

Age Makes a Big Difference

Older adults clear zolpidem much more slowly. In a study comparing elderly and younger subjects, the results were striking. Elderly men cleared the drug at roughly one-third the rate of younger men, their peak blood concentrations were more than double, and the half-life nearly doubled (2.7 hours versus 1.5 hours). For elderly women, clearance was about half that of younger women, and peak concentrations were also significantly higher.

The FDA’s guidance is straightforward: don’t take Ambien unless you have a full 7 to 8 hours of sleep ahead of you. For the middle-of-the-night formulation (Intermezzo), you need at least 4 hours remaining before you plan to be active. These aren’t conservative estimates. They reflect the reality that a meaningful percentage of people, particularly women and older adults, still have enough zolpidem in their blood to impair driving and reaction time after shorter sleep windows.

Even at the 8-hour mark, some people will have residual drug levels. The 15% of women and 3% of men who exceeded 50 ng/mL at eight hours in clinical trials weren’t just showing a lab curiosity. That concentration is associated with measurable impairment in driving simulator studies. If you feel groggy, slow, or “off” the morning after taking Ambien, the drug likely hasn’t fully cleared your system yet, regardless of what the clock says.
