A baby’s earlobe piercing takes about 6 to 8 weeks for initial healing and 3 to 4 months to fully strengthen. During that time, the original studs need to stay in place, and you’ll need to keep the area clean daily. Initial Healing vs. Full Healing

How Long to Keep the First Earrings In
The original piercing studs should stay in for at least four to six weeks without being removed or changed. Taking them out earlier, even briefly, risks the hole closing or becoming irritated. Babies’ earlobes heal quickly on the surface, which can be misleading. The channel underneath needs continuous support from the jewelry to form properly.
After that minimum window, you can try switching to a different pair, but handle the new earrings with clean hands and choose the same hypoallergenic material. If you notice any resistance or discomfort when inserting the new pair, it may be worth waiting another week or two.
Daily Cleaning Routine
The Association of Professional Piercers recommends using packaged sterile saline labeled for wound care. The only ingredient should be 0.9% sodium chloride. Contact lens saline, eye drops, and other saline products aren’t the same thing and shouldn’t be used on a piercing.
Rinse the front and back of each earlobe with the saline as needed. You don’t need to rotate the jewelry. This is an outdated recommendation that can actually pull bacteria into the healing channel and cause irritation. Just let the saline do the work, and gently pat the area dry with a clean paper towel or gauze.
Avoid cleaning with rubbing alcohol, hydrogen peroxide, antibacterial soaps, iodine, or products containing benzalkonium chloride (the active ingredient in many “pierced ear care solutions” sold alongside piercing kits). These are too harsh for healing tissue, especially on a baby’s sensitive skin, and they can slow the process down.
Choosing the Right Earring Material
Nickel is the most common trigger for contact dermatitis around piercings, and many inexpensive earrings contain it. For a baby, two materials stand out as the safest choices: medical-grade titanium and medical-grade plastic. Both are lightweight, durable, and hypoallergenic.
Medical-grade plastic is especially popular for first piercings because it’s affordable and very gentle on sensitive skin. Medical-grade titanium is a good long-term option as your child grows, since it comes in a wider variety of styles. Surgical steel is often marketed as hypoallergenic, but some grades still contain trace nickel, so titanium or plastic is the safer bet for a baby’s first pair.
Signs of Infection to Watch For
Some redness and mild tenderness in the first week or two is part of normal healing. An infection looks different. The key warning signs are:
- Discharge: Yellow, green, or foul-smelling pus coming from the piercing site. A small amount of clear or slightly white fluid can be normal, but colored or smelly discharge is not.
- Increasing redness and swelling: Redness that spreads outward from the piercing or swelling that gets worse instead of better over several days.
- Warmth: The earlobe feels noticeably hot to the touch compared to surrounding skin.
- Fever or chills: Any fever in a baby with a recent piercing warrants prompt medical attention, as it can signal the infection is spreading beyond the local area.
Minor irritation from bumping, pulling, or sleeping on the ear often mimics early infection symptoms. The distinguishing factor is usually the discharge. If you see colored pus or smell something off, that points toward infection rather than simple irritation.
Keloid Risk in Children
Keloids are raised, thickened scars that grow beyond the borders of the original wound. They’re caused by excessive scar tissue formation and are more common in people with darker skin pigmentation, including children of African American, Asian, and Hispanic descent. The good news is that most keloids develop after puberty, so the risk in babies and young children is relatively low.
If your family has a history of keloid scarring, it’s worth discussing this with a pediatrician before piercing. There’s no guaranteed prevention method, but knowing the risk ahead of time lets you make an informed choice and watch for early thickening around the site during healing.
Age and Readiness
There’s no strict minimum age for ear piercing from a medical standpoint. Ears can be pierced at any age, and when performed carefully with proper aftercare, the risk is low regardless of the child’s age. That said, guidance from pediatric health organizations suggests considering whether your child is old enough to help care for the piercing site themselves, since that reduces the chance of complications from touching, pulling, or forgetting aftercare.
For babies and toddlers, the aftercare responsibility falls entirely on you. This means consistently cleaning the piercings, keeping your baby from grabbing at the earrings, and monitoring for any signs of a reaction.
