Basaglar provides steady blood sugar control for a full 24 hours after each injection. This long-acting insulin works without a sharp peak, delivering a relatively flat effect throughout the day and night. Because it covers a full day, most people take it once daily at the same time.
How Long Basaglar Works in Your Body
After you inject Basaglar under the skin, it forms tiny clusters that dissolve slowly into your bloodstream over the course of the day. The glucose-lowering effect begins within a few hours and sustains itself for approximately 24 hours. The median time to its maximum effect is about 12 hours, but there’s no pronounced spike. Instead, you get a smooth, sustained baseline of insulin that mimics what a healthy pancreas releases between meals and overnight.
This 24-hour window is why Basaglar is dosed once a day. Some people split it into two daily doses 12 hours apart if directed by their prescriber, but a single injection covers most people’s basal insulin needs. When you first start taking it, the drug reaches a stable level in your system within 2 to 4 days of daily injections. Until that point, your blood sugar control may still be adjusting.
Injection Site Doesn’t Change the Duration
You can inject Basaglar into the abdomen, thigh, or upper arm. Clinical studies found no meaningful difference in how much insulin is absorbed or how long it lasts across these three sites. That said, rotating your injection site within the same general area helps prevent the skin from thickening over time, which could eventually slow absorption.
How Basaglar Compares to Lantus
Basaglar is a biosimilar to Lantus, meaning it contains the same active ingredient (insulin glargine) and performs the same way in the body. Clinical trials in adults with both type 1 and type 2 diabetes found the two equally safe and effective. The American Diabetes Association does not recommend one over the other. In practical terms, the duration, onset, and glucose-lowering profile are the same. If you’re switching between them, you can expect identical timing.
How Long a Basaglar Pen Lasts After Opening
Once you open a Basaglar KwikPen, it’s good for 28 days stored at room temperature (up to 86°F or 30°C). After 28 days, discard the pen even if insulin remains inside, because the medication may no longer work as intended. Unopened pens should be kept in the refrigerator until you’re ready to use them.
Cold insulin can sting more on injection, so many people take a new pen out of the fridge and let it reach room temperature before the first use. Just don’t leave an unopened pen at room temperature indefinitely, as refrigeration is what preserves it long term.
What to Do If You Miss a Dose
Because Basaglar’s effect tapers after 24 hours, a missed dose can leave a gap in your basal coverage. If you realize you’ve missed a dose, take it as soon as you remember, then return to your regular schedule. Never double up to compensate for a missed injection. If you frequently forget, setting a daily alarm or tying the injection to a consistent part of your routine (like brushing your teeth before bed) can help you stay on track.
