The smell that follows a Brazilian Butt Lift (BBL) typically lasts around 1 to 3 weeks, with the strongest odor occurring in the first week when fluid drainage and compression garment wear are at their peak. Most patients notice the smell fading significantly once drainage slows down, usually within 48 to 72 hours after surgery, and continuing to improve as healing progresses and garment hygiene becomes easier to manage.
What Causes the Smell
BBL smell comes from a combination of factors that all converge during early recovery: fluid drainage from incision sites, bacterial overgrowth on the skin, moisture trapped under a tight compression garment, and sweat. Your body is working overtime to heal, and the compression garment (often called a faja) creates a warm, moist environment that bacteria thrive in. When you add in the blood-tinged fluid that seeps from small incision sites, you get the conditions for a noticeable odor.
The bacteria driving the smell are mainly anaerobic species, the kind that grow in low-oxygen environments like the space between your skin and a snug garment. These bacteria break down proteins in drainage fluid and dead skin cells, releasing sulfur-containing compounds that produce a distinct, musty, sometimes sour smell. Aerobic bacteria on the skin’s surface contribute too, but the anaerobic species are the primary culprits behind the stronger odors.
The Typical Timeline
The smell tends to follow the same arc as your drainage. In the first 24 to 48 hours, drainage is heaviest and the odor is most noticeable. Without surgical drains, fluid leakage usually reduces significantly within 48 to 72 hours. If your surgeon placed drains, they’re typically removed within three to seven days, depending on how much fluid is still collecting.
For most patients, the worst of the smell lines up with this drainage window: the first 3 to 5 days. After that, the odor gradually fades as the incision sites close, drainage slows to almost nothing, and you’re better able to clean yourself and your garment regularly. By weeks 2 to 3, if you’re keeping up with hygiene, most patients report the smell is gone or barely noticeable. Some people with larger volume procedures or slower healing may notice a mild odor lingering into week 4, but this is less common.
Normal Smell vs. Warning Signs
A mild to moderate musty or sweaty smell during the first couple of weeks is normal and expected. It’s unpleasant but not dangerous. What you want to watch for is a smell that gets suddenly worse after it had been improving, or a distinctly foul, rotten odor that’s markedly different from the general “recovery funk.”
Infection and fat necrosis (where some of the transferred fat cells die) can both produce stronger, more pungent smells. But these conditions rarely show up as smell alone. They typically come with additional symptoms: redness spreading around an incision site, fever, increasing pain rather than decreasing pain, or a firm, tender lump developing under the skin. If you’re experiencing any of these alongside a worsening smell, that’s a different situation from the normal recovery odor and warrants a call to your surgeon.
How to Minimize the Odor
The compression garment is the biggest controllable factor. Bacteria build up quickly in a garment worn 24 hours a day against healing skin. Washing your garment after every use prevents this buildup, which is why most surgeons recommend having at least two garments so you can rotate between them. Use a gentle detergent and cold water, skip fabric softener and bleach (both damage the compression fibers), and air dry by hanging or laying flat on a towel. If you must use a dryer, use the lowest heat setting.
For your skin, gentle cleansing is key. Avoid scented soaps, exfoliating products, and anything harsh on the surgical area. A mild, fragrance-free cleanser works best. Pat the area dry thoroughly before putting your garment back on, since leftover moisture is exactly what feeds bacterial growth. Some patients find that placing a thin layer of gauze or absorbent pads between the skin and the garment helps absorb drainage and reduces garment saturation, which cuts down on odor between washes.
Changing your absorbent pads frequently during the first few days, when drainage is heaviest, makes a significant difference. The longer drainage fluid sits against your skin and garment, the more opportunity bacteria have to produce odor. Even small steps like changing pads every few hours in the first 48 to 72 hours can keep the smell noticeably more manageable.
Factors That Make the Smell Last Longer
The volume of fat transferred plays a role. Larger procedures tend to produce more drainage, which means more opportunity for bacterial overgrowth and garment saturation. Patients who have difficulty keeping up with garment hygiene, whether because of limited mobility, pain, or lack of a second garment to rotate, often report the smell persisting longer.
Hot, humid weather and physical activity (even light movement) increase sweating under the garment and can extend or intensify the odor. If you’re recovering during summer months, staying in air-conditioned environments when possible helps. Smoking and certain medications that slow wound healing can also extend the drainage period, indirectly prolonging the smell.
