Bellafill is designed to be a permanent dermal filler, and tissue samples taken 10 years after injection have confirmed that its structural microspheres remain intact and integrated within healthy collagen. That said, the FDA’s official labeling states that long-term safety and effectiveness beyond five years has not been established. In practice, most people can expect results lasting five years or longer, with some needing occasional touch-ups depending on the treatment area.
Why Bellafill Lasts Longer Than Other Fillers
Most injectable fillers use hyaluronic acid, a substance your body naturally breaks down within 9 to 24 months. Bellafill works differently. It contains tiny plastic microspheres (polymethylmethacrylate, or PMMA) suspended in a bovine collagen gel. The collagen gel provides immediate volume but gets absorbed by your body within about a month. During that time, the PMMA microspheres trigger your skin to produce its own new collagen around each microsphere.
By three months, each microsphere is encapsulated by your body’s own collagen fibers and the cells that produce them. The microspheres themselves are sized between 30 and 50 microns, which is large enough that your immune cells can’t break them down, and their smooth, round shape minimizes inflammatory reactions. A tissue study at the 10-year mark showed these microspheres still sitting within vascularized, mature collagen, essentially woven into the structure of the skin.
This is why Bellafill produces results that actually improve over time. Clinical observations in a 24-month study showed that skin severity scores continued to get better as months passed, reflecting the ongoing collagen-building effect rather than the gradual fading you’d see with a temporary filler.
Results for Nasolabial Folds vs. Acne Scars
For nasolabial folds (the lines running from the nose to the corners of your mouth), Bellafill typically provides correction lasting five years or more. The initial result comes partly from the collagen carrier gel, so you may notice a slight dip in volume during the first month as that gel absorbs. The new collagen your body builds then fills in and maintains the correction long term.
For acne scars, results may be effectively permanent. Bellafill doesn’t just fill the depression of a scar. It can stimulate enough collagen turnover to remodel the scarred tissue itself. Because acne scars are static depressions rather than dynamic folds that deepen with facial movement and aging, the correction tends to hold especially well.
Touch-Up Treatments
Most people need one or two touch-up sessions spaced at least two weeks apart to achieve their desired correction after the initial treatment. The manufacturer recommends filling no more than 100% of the skin defect in a single session, so building up gradually is standard practice. How many sessions you need depends on the depth of the defect, where it is on your face, and how much movement that area of skin experiences.
Beyond the initial correction period, some people may want additional touch-ups years later. Aging continues regardless of the filler, so while the Bellafill itself remains in place, changes in facial fat, bone structure, and skin elasticity over time can shift the overall appearance. These age-related changes are separate from the filler wearing off.
Cost Compared to Temporary Fillers
Bellafill costs more per syringe than hyaluronic acid fillers like Juvederm, typically running $1,000 to $1,500 per syringe compared to $500 to $800 for Juvederm. But the math changes when you factor in longevity. Juvederm needs to be repeated every 9 to 24 months. Over five years, you could easily spend several thousand dollars maintaining a Juvederm result in the same area where a single round of Bellafill would still be holding.
The Permanence Trade-Off
The same quality that makes Bellafill last so long also creates a significant downside: it cannot be dissolved. Hyaluronic acid fillers can be reversed with an enzyme injection if something goes wrong or you don’t like the result. Bellafill has no equivalent reversal option. The microspheres become integrated into your tissue with collagen and scar tissue growing around them, making clean removal difficult.
If removal becomes necessary, it requires open surgical excision. Even then, the filler can often only be partially removed. Surgeons have to balance taking out the material against creating new contour deformities or risking nerve injury. One case review described the process as significantly more challenging than the original placement, with the integration of filler into surrounding tissue making complete excision impractical in many cases.
This is why most providers recommend a skin test before the first treatment (to check for allergic reactions to the bovine collagen component) and a conservative, gradual approach to injection. Starting with less and adding more is straightforward. Removing an overcorrection is not.
Who Gets the Longest-Lasting Results
People treating static problems, like acne scars or deep-set nasolabial folds, tend to see the most durable outcomes. Areas with significant movement or ongoing age-related volume loss may still look different over time, not because the Bellafill has broken down, but because the surrounding anatomy has changed. Younger patients with good skin quality and a specific, well-defined defect to correct are generally the best candidates for truly long-lasting results.
