Phenylbutazone, commonly called “bute,” typically begins reducing pain and inflammation in horses within a few hours of administration, with peak effects building over the first 12 to 24 hours depending on the dose. A single standard dose provides meaningful relief for roughly 12 to 24 hours, which is why most horses receive it once or twice daily.
Onset and Duration of a Single Dose
Bute works by blocking the enzymes that produce prostaglandins, the chemicals responsible for pain, swelling, and inflammation at an injury site. Once absorbed, it begins suppressing those inflammatory signals fairly quickly, but the full clinical effect takes time to build. In studies on experimentally induced joint inflammation in horses, a standard dose of 2 mg/kg produced measurable pain relief lasting 8 to 16 hours. Doubling that dose extended the duration to around 14 hours, and a higher dose of 4.4 mg/kg kept inflammatory markers suppressed for a full 24 hours.
In practical terms, most horse owners and veterinarians report noticeable improvement within 1 to 2 hours after oral dosing, with the horse moving more comfortably and showing less visible lameness. The drug is largely metabolized within about 8 hours, but its anti-inflammatory effects outlast its presence in the bloodstream. That’s an important distinction: just because the drug is being cleared doesn’t mean the pain relief has worn off.
Research on horses with navicular disease found that once-daily dosing at 4.4 mg/kg improved weight-bearing on the forelimbs for a full 24 hours after the last dose. At that same dose, visible lameness scores improved for about 12 hours. So while bute is clearly working within hours, the degree of improvement you see depends on the severity of the problem and how you’re measuring it.
Standard Dosing for Horses
The typical dose ranges from 1 to 2 grams per 1,000 pounds of body weight daily. For oral paste or powder, common dosing falls between 2.2 and 4.4 mg/kg per day, often split across the day. The FDA-approved label for injectable bute recommends dividing the daily dose into thirds given at 8-hour intervals for more consistent pain control.
Many veterinarians start at the higher end of the range for the first day or two to get inflammation under control, then taper to the lowest effective dose. Injectable bute should not be given intravenously for more than 5 consecutive days before switching to oral forms. Oral bute can be used longer, but extended use carries real risks.
How Long Until You See a Real Difference
For acute injuries like a soft tissue strain or a bruised sole, you’ll often see a clear improvement within the first dose or two. The horse bears weight more evenly, moves with less stiffness, and may eat and drink more normally. For chronic conditions like arthritis or navicular syndrome, it can take 2 to 3 days of consistent dosing before the full benefit becomes obvious, because the drug needs time to bring down the accumulated inflammatory response.
If your horse hasn’t shown any improvement after 3 to 4 days on bute at an appropriate dose, that’s worth discussing with your vet. It may mean the pain source is something bute doesn’t address well, or the underlying problem is more serious than initially suspected.
Risks of Prolonged or High-Dose Use
Bute is effective, but it’s not harmless. In research, all horses given phenylbutazone at high doses developed significant gastric disease in the glandular portion of the stomach. This is the lower, more acid-producing part of the equine stomach, and it’s particularly vulnerable to the effects of anti-inflammatory drugs that suppress protective prostaglandins.
The more serious risk with extended or excessive use is damage to the large colon, particularly a condition called right dorsal colitis. This involves ulceration of the colon lining, which leads to protein loss, diarrhea, ventral edema (fluid swelling along the belly), weight loss, lethargy, and recurring colic. In severe cases, the intestinal wall breaks down enough to allow bacteria into the bloodstream, causing fever, dehydration, and rapid heart rate. Signs can appear days to weeks after starting treatment, and scarring of the colon can cause impactions that sometimes require surgery.
Early warning signs of toxicity include difficulty chewing or swallowing (from mouth or esophageal ulcers), excessive drooling, lying down after eating, loss of appetite, and soft or watery manure. If you notice any of these while your horse is on bute, stop the drug and contact your veterinarian.
Competition Withdrawal Times
If your horse competes, bute must be out of the system before show day. The drug can be detected in blood for approximately 84 hours (about 3.5 days) after administration using standard testing methods. However, the recommended withdrawal time for competition horses is 5 days from the last dose. This buffer accounts for individual variation in how quickly horses metabolize the drug and provides a safety margin against a positive test.
Keep in mind that different governing bodies may set slightly different thresholds and withdrawal guidelines. Always check the specific rules for your discipline and sanctioning organization, as penalties for a positive test can be severe regardless of intent.
Why Bute Is Only for Horses
Phenylbutazone was originally developed for humans in the early 1950s as an arthritis drug. By the mid-1980s, it had been banned from human use in most Western countries. The reason: it caused rare but fatal blood disorders, including aplastic anemia (a collapse in blood cell production) and agranulocytosis (a dangerous drop in white blood cells). Internal documents from its manufacturer estimated the drug had caused nearly 1,200 deaths, while UK regulators put the figure closer to 1,500. It remains available only for horses and is not safe for human consumption, which is also why bute-treated horses cannot enter the food chain.
