How Long Does Cephalexin Stay in Your System?

Cephalexin is eliminated from your body quickly. With a half-life of roughly 1 hour, most of the drug clears your system within 6 to 8 hours of your last dose. This makes it one of the faster-clearing antibiotics available, which is also why it needs to be taken multiple times a day during treatment.

How Quickly Cephalexin Leaves Your Body

After you swallow a dose of cephalexin, it reaches its highest concentration in your blood in about 1 hour. From that point, levels drop rapidly. The elimination half-life, meaning the time it takes for your body to clear half the drug from your bloodstream, is approximately 58 minutes in a healthy adult with normal kidney function.

A drug is generally considered fully eliminated after 5 to 6 half-lives. For cephalexin, that works out to roughly 5 to 6 hours. Research confirms this timeline: 70 to 100% of a dose can be recovered in urine within 6 to 8 hours. So if you take your last dose at 8 a.m., cephalexin is essentially gone from your system by mid-afternoon the same day.

How Your Body Processes It

Cephalexin is unusual in that your body barely metabolizes it. About 90% of the drug is excreted unchanged through the kidneys directly into your urine. Your liver does very little work breaking it down. This means the speed at which cephalexin leaves your body depends almost entirely on how well your kidneys are functioning.

Kidney Function Changes the Timeline Significantly

Because the kidneys do nearly all the work of clearing cephalexin, any reduction in kidney function slows elimination dramatically. In a healthy adult, the half-life sits around 58 minutes. In someone with no kidney function at all, that half-life stretches to 15.4 hours, according to pharmacokinetic research published in the journal Antimicrobial Agents and Chemotherapy. At that rate, the drug could take 3 to 4 days to fully clear.

People with moderate kidney impairment fall somewhere between those two extremes. If you have chronic kidney disease or any condition that reduces your kidney function, cephalexin will stay in your system longer than the standard 6 to 8 hours. This is one of the reasons doctors adjust dosing schedules for people with kidney problems.

Clearance in Newborns and Infants

Newborns clear cephalosporin antibiotics (the drug class cephalexin belongs to) more slowly than older children and adults. This is because a newborn’s kidneys are still maturing, so their filtration rate is lower. As infants grow over their first weeks and months, kidney function ramps up and drug clearance speeds up to match adult rates. Older infants and children typically clear these drugs at rates comparable to, or even faster than, adults.

Cephalexin in Breast Milk

If you’re breastfeeding and taking cephalexin, the drug does pass into breast milk in small amounts. After a single 1-gram dose, peak milk levels occurred 4 to 5 hours later, averaging 0.51 mg/L. That’s a very low concentration. After a 500 mg dose, peak milk levels reached about 0.7 mg/L at the 4-hour mark. In a woman taking 500 mg four times daily over 16 days, milk levels ranged from roughly 0.4 to 1 mg/L, with little variation tied to when doses were taken. These concentrations are low enough that cephalexin is generally considered compatible with breastfeeding.

Why the Short Half-Life Matters for Dosing

Cephalexin’s 1-hour half-life is the reason it’s typically prescribed to be taken every 6 to 12 hours. For the antibiotic to work, its concentration in your blood needs to stay above a certain threshold long enough to kill bacteria. Because the drug clears so fast, missing doses or spacing them too far apart can let blood levels drop below that effective range, giving bacteria a chance to survive and potentially develop resistance.

Some prescribers pair cephalexin with probenecid, a medication that slows kidney excretion of the antibiotic. This combination has been shown to increase cephalexin’s effective duration dramatically, keeping blood levels high enough throughout a 6-hour dosing window where cephalexin alone would fall short.

Drug Testing and Interactions

Cephalexin is not a controlled substance and is not part of standard drug screening panels. However, cephalosporins can occasionally cause a false positive on certain urine glucose tests (the type used to monitor diabetes, not workplace drug tests). If you’re monitoring urine glucose and taking cephalexin, be aware that results may be inaccurate until the drug has cleared, typically within 8 hours of your last dose.

Because cephalexin exits the body so quickly, it’s unlikely to interact with medications or procedures scheduled more than a day after your final dose. For practical purposes, the drug is gone from your system within a single day of stopping treatment.
