Cialis (tadalafil) lasts up to 36 hours after a single dose, far longer than any other erectile dysfunction medication on the market. That wide window is why it’s sometimes called “the weekend pill.” But the full picture involves when it kicks in, what affects its duration, and how the daily version works differently.
The 36-Hour Window
Clinical studies consistently show that a single 10 mg or 20 mg dose of Cialis can support erectile function for up to 36 hours. That doesn’t mean you’ll have an erection for 36 hours. It means that during that window, if you become sexually aroused, the medication is available in your system to help you get and maintain an erection. In men over 65, studies found that at least 56% of attempts at intercourse were successful at all time points up to 36 hours after taking the pill, confirming the drug works across that full window regardless of age.
The reason Cialis lasts so much longer than alternatives comes down to how slowly your body clears it. Tadalafil has a half-life of 17.5 hours, meaning it takes that long for your body to eliminate just half the dose. By comparison, sildenafil (Viagra) has a half-life of about 4 hours, and vardenafil (Levitra) sits at 4 to 6 hours. That’s why Viagra’s effects typically fade after 4 to 6 hours while Cialis keeps working well into the next day.
How Quickly It Kicks In
Most men notice effects within 30 to 60 minutes of taking Cialis, though some clinical research has tested for onset as early as 30 minutes. There’s one catch worth knowing: eating a meal before taking it can delay peak effectiveness by roughly an hour and a half. While older studies suggested food had no meaningful impact on tadalafil, more recent research from the Journal of Personalized Medicine found that volunteers who took it after eating reached peak blood levels notably later than those who took it on an empty stomach. If timing matters to you, taking it before a meal will get it working faster.
Because of its long duration, though, many men take Cialis earlier in the day and don’t worry about precise timing the way they might with a shorter-acting pill.
Daily Dosing vs. As-Needed
Cialis comes in two different approaches. The as-needed version (10 mg or 20 mg) is taken before anticipated sexual activity, and its effects can last longer than 24 hours. Because of that long tail, you shouldn’t take another as-needed dose within 24 hours of the first.
The daily version uses a lower dose, typically 2.5 mg or 5 mg, taken at the same time every day regardless of when you plan to have sex. After a few days of consistent use, the drug builds to a steady level in your bloodstream, so you don’t need to plan around taking a pill. This approach is also prescribed for men who have both ED and an enlarged prostate, since tadalafil helps with urinary symptoms too. For men who have sex frequently, the daily option removes the need to think about timing altogether.
How It Compares to Other ED Medications
- Sildenafil (Viagra): Lasts 4 to 6 hours. Works faster on an empty stomach and is more sensitive to food. Best for men who want a shorter, more defined window.
- Vardenafil (Levitra): Lasts 4 to 8 hours with a half-life of 4 to 6 hours. Similar to sildenafil in duration.
- Tadalafil (Cialis): Lasts up to 36 hours. Less affected by food than sildenafil, though eating can still delay onset. Best for men who prefer spontaneity.
The tradeoff for Cialis’s longer duration is that side effects, if they occur, also stick around longer. A headache from Viagra might fade in a few hours, while one from Cialis could linger.
How Long Side Effects Last
The most common side effects are headaches, facial flushing, muscle aches (particularly in the back and legs), and nasal congestion. These tend to be mild and follow a predictable pattern. Headaches are most common when you first start taking the medication and typically go away after the first week of use. Flushing usually resolves within a few days. Muscle aches, which are somewhat unique to tadalafil compared to other ED pills, can show up 12 to 24 hours after a dose and generally pass within a day or two.
If you stop taking Cialis, side effects clear once the drug leaves your system. Given the 17.5-hour half-life, that means the medication is mostly gone within two to three days after your last dose.
What Can Shorten or Extend the Duration
Several factors influence how long you personally feel the effects. Body weight plays a role: the drug is distributed through your body’s tissues, so larger individuals may metabolize it differently. Liver and kidney function matter too, since both organs are involved in clearing tadalafil from your system. Reduced function in either can slow elimination, effectively extending the drug’s duration and intensity.
Alcohol can reduce the quality of erections independently of the medication, making it seem like the drug wore off sooner than it did. And while age doesn’t appear to significantly shorten the 36-hour window based on clinical data, older men may find the onset slightly slower or the effect less pronounced compared to younger men on the same dose.
