How Long Does Clomid Stay in Your System?

Clomid (clomiphene citrate) can stay in your system for several weeks after your last dose. That’s because the drug is actually a mix of two compounds that leave your body at very different speeds: one clears within a couple of days, while the other lingers for months. The answer depends on which component you’re asking about and why it matters to you.

Two Compounds, Two Very Different Timelines

Clomiphene citrate contains two mirror-image molecules called isomers. Enclomiphene, the one responsible for most of the fertility effects, has a half-life of about 10.5 hours. That means half of it is gone from your blood in under half a day, and it’s essentially cleared within two to three days.

Zuclomiphene is a different story. It has a half-life of roughly 30 days. With each half-life, the amount in your body drops by half, but it takes five to six half-lives for a drug to be considered fully eliminated. That puts zuclomiphene’s total clearance time at around five to six months after your last dose. This is why traces of clomiphene can be detected in the body long after you stop taking it, even though you may feel its effects wear off much sooner.

How Your Liver Processes Clomid

Your liver breaks down clomiphene primarily through a specific enzyme called CYP2D6. The speed of this process varies from person to person because CYP2D6 activity is genetically determined. Some people are “extensive metabolizers” who clear the drug efficiently. Others are “poor metabolizers” whose livers process the drug much more slowly. In lab studies using liver tissue from poor metabolizers, researchers found essentially no breakdown of the drug at all.

This genetic variability means two people taking the same dose of Clomid can have meaningfully different blood levels and clearance times. The World Anti-Doping Agency (WADA) classifies clomiphene as a prohibited substance because it can boost testosterone levels in men. Their testing guidelines note that the parent compound can be detected in urine for a prolonged period during use, followed by a rapid drop once you stop. However, because zuclomiphene has such a long half-life, traces and metabolites can appear in urine samples well after the last dose.

WADA also monitors specific breakdown products of clomiphene that have even longer detection windows than the parent drug. Standard workplace drug panels do not test for clomiphene, so this is primarily a concern for competitive athletes subject to anti-doping protocols.

When Side Effects Fade

Common side effects of Clomid, including hot flashes, bloating, mood changes, and visual disturbances like blurring or light sensitivity, are generally mild and resolve quickly once you stop taking the drug. For most people, these effects track more closely with enclomiphene’s rapid clearance than with zuclomiphene’s slow exit. You can expect the typical side effects to fade within days of your last dose, not weeks.

Visual side effects deserve specific attention. While they’re uncommon, symptoms like blurred vision, spots, or flashes of light should prompt you to stop the medication. These visual changes usually resolve after discontinuation, but their presence can indicate sensitivity that your prescriber should know about.
