This difference matters most in the first two days after surgery, when pain tends to be worst and opioid use is highest. By covering that critical window with a local anesthetic, Exparel can reduce your reliance on oral or IV opioids during the period when side effects like nausea, sedation, and constipation are most disruptive to recovery.
Safety Considerations
Because Exparel releases bupivacaine slowly into surrounding tissue, it carries the same risks as any local anesthetic, including a rare but serious complication called local anesthetic systemic toxicity. This occurs when too much of the drug enters the bloodstream, potentially causing seizures or cardiac problems. The risk is highest if the medication is accidentally injected into a blood vessel rather than the intended tissue.
The maximum recommended dose is 266 mg (one 20 mL vial of undiluted product) for adults. For children aged 6 and older, the dose is weight-based at 4 mg per kilogram, capped at the same 266 mg maximum.
