How Long Does Fat Transfer Last in Your Face?

Fat transferred to your face is permanent once it fully integrates, but not all of the injected fat survives. Studies show that between 30% and 83% of grafted fat cells survive in the facial area, with the wide range depending on technique, surgeon skill, and individual biology. The fat that does establish a blood supply and “take” in its new location remains there indefinitely, behaving like the fat cells that were already in your face. The body’s immune cells gradually clear away this dead tissue, and the space either shrinks or fills with scar tissue. This is why every fat transfer loses some volume. It’s a biological inevitability, not a sign something went wrong.

The size of each deposited fat droplet matters enormously. Modeling shows that a fat deposit with a radius larger than about 1.6 millimeters will develop a necrotic core. When large clumps of fat are allowed to pool together, the result is extensive cell death, possible cyst formation, and significant volume loss that can continue for up to a year. This is why experienced surgeons inject fat in tiny, carefully spaced droplets rather than large deposits.

The Timeline From Surgery to Final Result

Your face will look overfilled immediately after the procedure, and that’s intentional. Surgeons typically overcorrect because they know a portion of the fat won’t survive. Here’s roughly what to expect:

  • Week 1: Peak swelling and bruising. Your face will look noticeably fuller than the intended result.
  • Weeks 2 to 4: Fat cells that failed to establish blood supply are gradually absorbed. Swelling begins to subside.
  • Weeks 4 to 12: The tissue softens and settles. You’ll start to see a more realistic picture of your final outcome around the 6 to 8 week mark.
  • Months 3 to 6: Most resorption is complete. The volume you see is close to what you’ll keep.
  • Months 6 to 12: If larger deposits were used, some continued absorption can occur. By one year, the result is fully stable.

The surviving fat cells are now part of your facial tissue. They have their own blood supply, they respond to hormones, and they behave like native fat. If you gain or lose significant weight, these cells will grow or shrink just like fat anywhere else in your body.

Why Survival Rates Vary So Much

That 30% to 83% survival range is wide because many factors influence how much fat takes. Some are within the surgeon’s control, others aren’t.

Technique is the biggest variable. Fat can be harvested and processed as macrofat (large particles over 2.4 mm), microfat (particles under 1.2 mm), or nanofat (particles between 0.4 and 0.6 mm). For facial work, microfat is generally preferred because the smaller particle size allows better oxygen diffusion and fits the delicate contours of the face. Once the surviving fat has integrated (typically by six months), those results are considered permanent. You won’t wake up one day and find the volume has disappeared. The transferred fat cells are living tissue with a blood supply, not a filler that degrades over time.

That said, “permanent” doesn’t mean “unchanging forever.” Your face continues to age. You’ll still lose bone density, collagen, and native fat over the years, which can gradually alter the appearance of your results. Weight fluctuations affect transferred fat just as they affect fat elsewhere. Significant weight loss can reduce the volume in your face, while weight gain can increase it.

Some people choose to have a second round of fat transfer if the first round didn’t produce enough surviving volume. This is common and not a sign of failure. Given that a substantial percentage of transferred fat is always lost, touch-up procedures are a normal part of the process. Most surgeons recommend waiting at least six months before evaluating whether additional grafting is needed, since that’s when the final volume stabilizes.

Fat transfer involves a more involved initial procedure (liposuction to harvest the fat, then injection into the face), but the results that survive don’t need ongoing maintenance.

Fat also integrates more naturally into facial tissue because it is your own tissue. It moves with your expressions and feels soft. The tradeoff is less predictability. With filler, what you see immediately is close to what you get. With fat, you’re waiting months to learn your final result, and the degree of volume retention varies from person to person.
