What Happens When Ear Crystals Come Loose

Your inner ear contains tiny calcium carbonate crystals called otoconia, which sit on a sticky gel pad inside a structure called the utricle. When these crystals break free and drift into one of the semicircular canals (the fluid-filled loops that detect head rotation), they send false signals to your brain every time you move your head. The result is benign paroxysmal positional vertigo, or BPPV: brief but intense spinning sensations triggered by rolling over in bed, looking up, or tilting your head.

Once crystals are floating loose in a canal, two things can happen. They can drift back into the utricle on their own, or they can gradually dissolve in the surrounding fluid. The fluid inside the semicircular canals is slightly acidic compared to the crystals’ calcium carbonate composition, which slowly breaks them down. This natural dissolution process is one reason BPPV often resolves without any treatment at all.

The Natural Resolution Timeline

For most people, BPPV disappears within a few days of the first episode. A smaller group needs several weeks for the crystals to either dissolve or migrate out of the canal. The Cleveland Clinic notes that if symptoms haven’t cleared within six weeks, it’s reasonable to seek treatment rather than continuing to wait.

The timeline varies because several factors influence how quickly the crystals break down. The acidity of the inner ear fluid fluctuates throughout the day, and crystals erode faster during periods of lower pH. This is actually why BPPV episodes are so common in the morning: the fluid becomes slightly more acidic overnight, which can fragment crystals and stir up debris right before you wake up and start moving your head.

Calcium metabolism also plays a role. The calcium concentration in your inner ear fluid mirrors what’s in your blood. As people age, calcium absorption in the gut decreases, which changes the chemical environment of the inner ear in two ways: it makes the crystals themselves more fragile and prone to breaking off, but it also alters the fluid chemistry that governs how quickly loose fragments dissolve. This is one reason BPPV becomes more common with age and why some episodes resolve faster than others.

How Repositioning Treatment Works

If you’d rather not wait weeks for crystals to dissolve on their own, a repositioning maneuver can fix the problem in minutes. The most common version, the Epley maneuver, involves a specific sequence of head positions that use gravity to guide the loose crystals out of the semicircular canal and back into the utricle, where they belong. No medication, no surgery.

The success rate is high. In clinical studies, about 63% of patients had their crystals successfully repositioned on the first attempt with the standard Epley maneuver. A modified version of the technique pushed that first-attempt success rate to 85%. Even when the first session doesn’t fully work, a second or third attempt usually does. Your doctor can confirm whether crystals are still displaced using a diagnostic test called the Dix-Hallpike maneuver, where they move your head into a specific position and watch your eyes for involuntary movement. Residual Dizziness After Crystals Are Gone

Here’s something that catches many people off guard: you can still feel dizzy even after the crystals have been successfully repositioned or dissolved. This residual dizziness is common, and it doesn’t mean the treatment failed or that crystals are still loose.

The cause is your brain recalibrating. While crystals were sending faulty signals, your brain adapted to that incorrect input. Once the crystals are gone and normal signals resume, your brain needs time to readjust. This central compensation process varies widely from person to person. Some people feel completely normal within days, while others experience a vague unsteadiness or motion sensitivity for weeks. In cases where the underlying balance organ sustained some damage (not just displaced crystals), residual symptoms can persist even longer, since repositioning maneuvers can’t repair tissue damage.

Recurrence and Prevention

BPPV has a notable recurrence rate. About 34% of patients experience another episode within a year of their initial resolution, based on follow-up data from a study of 122 patients. The crystals can break loose again for the same reasons they displaced in the first place: aging, head trauma, prolonged bed rest, or changes in calcium metabolism.

Vitamin D levels appear to play a role in who develops BPPV in the first place. A large meta-analysis covering nearly 10,000 individuals found that people with lower vitamin D levels were more likely to develop BPPV than those with adequate levels. Whether low vitamin D specifically drives recurrence is less clear from the data, but supplementation in patients with confirmed deficiency has been shown to reduce the frequency of repeat episodes and prolong symptom-free periods. Since vitamin D is essential for calcium regulation throughout the body, including the inner ear, keeping your levels in a healthy range is a reasonable preventive step, particularly if you’ve already had BPPV once.

Staying active also helps. Regular head movement and physical activity support the brain’s vestibular compensation system, making it faster to adapt if crystals do come loose again. Prolonged bed rest or avoiding head movements out of fear of triggering vertigo can actually slow recovery and make recurrences feel worse when they happen.
