But the number on the scale can be misleading early on, because testosterone simultaneously builds lean muscle and can cause mild water retention in the first weeks. The First Few Weeks: Scale Weight May Not Budge

During the first month or so, don’t be surprised if the scale stays the same or even ticks upward. Testosterone commonly causes mild water retention when you first start, though the exact reason isn’t fully understood. This temporary fluid gain can mask any early fat loss. At the same time, testosterone begins shifting your body composition almost immediately, activating pathways in the liver that increase fat mobilization within minutes of exposure. But these cellular-level changes take weeks to translate into visible results.

Months 1 Through 4: When Fat Loss Becomes Measurable

Clinical studies consistently show a decrease in fat mass by three months of treatment, with some studies detecting it as early as 12 weeks. This is the window where most people first notice their clothes fitting differently or see changes in the mirror. A review published in the European Journal of Endocrinology found that changes in fat mass, lean body mass, and muscle strength all begin within this 12-to-16-week range.

What makes this phase tricky is that your body is gaining muscle at the same time it’s losing fat. Lean body mass increases by roughly 10% in the first three months. Since muscle is denser than fat, you can lose a noticeable amount of body fat while your total weight barely changes. This is why body measurements, progress photos, or body fat percentage are far more useful than the scale alone during this period.

Months 6 Through 12: Where Results Stabilize

After that point, marginal changes can still occur over years, but the most dramatic shifts in body composition happen in the first year. Testosterone Targets Visceral Fat Specifically

What a Realistic Timeline Looks Like

Putting it all together, here’s what to expect at each stage:

  • Weeks 1 to 4: Possible mild water retention. Fat metabolism is increasing at a cellular level, but visible changes are unlikely. Scale weight may go up slightly.
  • Weeks 12 to 16: Measurable decreases in fat mass and increases in lean mass. Clothes fit differently. Metabolic rate is roughly 10% higher than before treatment.
  • Months 6 to 12: Body composition changes approach their maximum. Visceral fat reduction is most pronounced. The scale may still not reflect the full picture because of simultaneous muscle gain.
