Most healthy couples conceive within 12 months of trying. For women in their early to mid-20s, the chance of getting pregnant in any given month is 25 to 30 percent, which means even under ideal conditions, it often takes several cycles. Your actual timeline depends on age, health, how well you time intercourse, and a few other factors worth understanding.
The General Timeline, Month by Month
Fertility isn’t a light switch. Each month represents a new opportunity with roughly the same odds, so conception follows a pattern of accumulating probability over time. Among couples who track ovulation and time intercourse well, the average time to pregnancy is about 4 cycles. Those with the strongest fertility indicators (under 35, regular cycles, frequent intercourse) reach an 88 percent pregnancy rate by 6 cycles and 96 percent by 12 cycles, with a median of just 2 cycles.
For couples who aren’t actively tracking ovulation, the timeline stretches. Without targeted timing, some months you may miss the fertile window entirely. The fertile window itself is narrow: roughly 5 days before ovulation and the day of ovulation, totaling about 6 days per cycle. Missing it means that month’s chance drops to zero regardless of how often you have sex outside that window.
The bottom line: if you’re under 35 and timing things reasonably well, there’s a strong chance you’ll conceive within 6 months. If it takes up to 12 months, that’s still considered normal.
How Age Changes the Odds
Age is the single biggest factor in how long it takes to get pregnant, and it affects both partners.
For women, the monthly conception rate starts high in the 20s (25 to 30 percent per cycle) and declines gradually through the 30s. By 40, the chance of conceiving in any given cycle drops to around 5 percent. That doesn’t mean pregnancy at 40 is impossible, but the math changes dramatically. At a 5 percent monthly rate, it can take considerably longer, and the cumulative 12-month probability is much lower than it is for someone a decade younger. This decline is driven primarily by a decrease in the number and quality of eggs over time.
Male age matters too, though the effect is less dramatic. Conception is about 30 percent less likely for men over 40 compared to men under 30. Sperm quality, including motility and DNA integrity, gradually declines with age. Couples where both partners are older may face compounding effects on their timeline.
Weight and Lifestyle Factors
Body weight has a measurable impact on how quickly couples conceive. Research on prepregnancy BMI found that the optimal range for women is roughly 21 to 23, while for men it’s about 23 to 28. When both partners fall outside normal ranges, fertility drops noticeably. Couples where both partners were obese had a 19 percent reduction in their monthly conception rate. Couples where both were underweight saw a 10 percent reduction. The most significant effect was seen when an obese woman was partnered with an underweight man, which reduced fertility by 30 percent.
Beyond weight, a few other lifestyle factors can extend your timeline. Smoking reduces fertility in both men and women. Heavy alcohol use (more than a few drinks per week) is linked to longer time to pregnancy. High stress levels can disrupt ovulation in women, though the research on stress is less precise. Regular moderate exercise generally supports fertility, but extreme endurance training can suppress ovulation in some women.
Coming Off Birth Control
If you’ve recently stopped contraception, your timeline partly depends on which method you were using. Fertility returns faster than most people expect with most methods, but there’s one notable exception.
After stopping the combination pill, patch, or vaginal ring, you can technically get pregnant right away. About half of women conceive within the first 3 months, and most within 12 months. After having an IUD removed, whether copper or hormonal, fertility typically returns with your very first menstrual cycle. The same is true for the implant: once it’s removed, you can conceive immediately. The progestin-only mini-pill is similar, with most women getting pregnant within 6 months of stopping.
The injectable shot is the outlier. Because the hormone is designed to release slowly from the injection site, it can take 3 to 18 months after your last shot for fertility to fully return. If you’re planning to start trying soon, this is worth factoring into your timing.
Tracking Ovulation to Shorten the Wait
One of the most effective things you can do is identify your fertile window each cycle. Women who track ovulation through apps, basal body temperature, cervical mucus patterns, or ovulation predictor kits conceive faster on average than those who don’t. In studies of app-based fertility tracking, the mean time to pregnancy was 4 cycles, and women with strong fertility indicators who tracked consistently had a median of just 2 cycles.
Ovulation predictor kits, available at any pharmacy, detect a hormone surge that happens 24 to 36 hours before ovulation. Having intercourse in the two days surrounding that surge gives you the best odds. You don’t need to have sex every day during the fertile window. Every other day is sufficient and produces similar conception rates. Cervical mucus is another reliable signal: when it becomes clear, stretchy, and slippery (often compared to egg whites), ovulation is approaching.
When the Timeline Feels Too Long
Fertility specialists use age-based thresholds to determine when a couple should be evaluated. For women under 35, the guideline is to seek an evaluation after 12 months of regular, well-timed intercourse without conceiving. For women 35 and older, that window shortens to 6 months. Women over 40 may benefit from more immediate evaluation, given the steeper decline in monthly conception rates.
These thresholds exist because infertility affects roughly 1 in 7 couples, and many underlying causes are treatable. About a third of cases involve a female factor, a third involve a male factor, and the remaining third are either a combination of both or unexplained. A basic fertility evaluation typically involves bloodwork to assess hormone levels and ovarian reserve for women, and a semen analysis for men. Both are straightforward, and identifying a specific issue early can save months of frustration.
If you have known risk factors, such as irregular or absent periods, a history of pelvic inflammatory disease, endometriosis, prior cancer treatment, or undescended testicles, it’s reasonable to pursue evaluation sooner rather than waiting out the full 6 or 12 month timeline.
