Because pharmacologists use a “five half-lives” rule to estimate full elimination, the math works out to approximately 11 to 12 days for the drug itself to drop below detectable levels, though traces of its breakdown products may linger slightly longer.
How Ivermectin Moves Through Your Body
Ivermectin is absorbed through the gut and reaches its peak concentration in your blood about 3.5 to 4 hours after you take it, though this can range anywhere from 2 to 9 hours depending on the individual. Taking it with food, especially a high-fat meal, increases absorption significantly.
Once absorbed, ivermectin is highly fat-soluble. It doesn’t just float around in your bloodstream. It spreads deeply into your tissues, concentrating especially in the liver, skin, and body fat. This large volume of distribution is one of the main reasons it takes so long to leave your system. The drug essentially parks itself in fatty tissue and then slowly releases back into the blood over days, which extends its overall presence in your body well beyond what you’d expect from the blood levels alone.
How Your Liver Breaks It Down
Your liver does the heavy lifting when it comes to processing ivermectin. A family of liver enzymes called CYP3A4 handles the bulk of the metabolism, with CYP3A5 playing a smaller supporting role. These enzymes break ivermectin into several inactive metabolites that are then shuttled out of the body.
This matters for two practical reasons. First, anything that slows down CYP3A4 can extend how long ivermectin stays in your system. Certain medications, particularly some antifungals, antibiotics, and HIV medications, are known CYP3A4 inhibitors. If you take one of these alongside ivermectin, the drug may clear more slowly and reach higher levels in your blood. Second, people with significant liver disease may process ivermectin less efficiently, potentially prolonging its effects.

Ivermectin exits almost entirely through your stool. About 98% of the administered dose, along with its metabolites, is excreted in feces over an estimated 12 days. Less than 1% leaves through urine. Then comes a slower terminal phase, with a half-life of roughly 53 hours, as the drug that was stored in fat and other tissues gradually works its way back into circulation and gets processed. This two-phase pattern is why you might see different half-life numbers cited in different sources: both are correct, they just describe different stages of the process.
Factors That Change How Long It Stays
Several things can shift these timelines in either direction:
- Body fat percentage. Because ivermectin accumulates in fatty tissue, people with higher body fat may retain the drug longer as it slowly releases from those stores.
- Liver function. Since CYP3A4 enzymes in the liver are responsible for breaking down ivermectin, any compromise in liver function can slow clearance.
- Other medications. Drugs that compete for or inhibit CYP3A4 can raise ivermectin levels and extend its time in your body. This includes some commonly prescribed antifungals and certain antibiotics.
- Dose size. Higher doses take longer to fully eliminate, simply because there’s more of the drug to process.

Why Repeat Doses Are Spaced Days Apart
If you’ve been prescribed ivermectin for a parasitic infection, you may have noticed that repeat doses are typically scheduled a week or more apart. For classic scabies, the CDC recommends two doses taken 7 to 14 days apart. For more severe crusted scabies, dosing schedules can stretch over weeks, with doses on specific days across a 29-day period.
These intervals aren’t based on how fast the drug leaves your system. They’re designed around the life cycle of the parasite. Ivermectin kills adult parasites and larvae effectively, but it doesn’t kill eggs. The gap between doses gives unhatched eggs time to develop into larvae, which the second dose then targets. The drug’s relatively long presence in your body actually works in your favor here, maintaining some level of activity between doses.
Detection in Blood Tests
In standard clinical scenarios, ivermectin isn’t part of routine drug screening panels. It won’t show up on a workplace drug test. However, if someone specifically tests for it using liquid chromatography or mass spectrometry, ivermectin is typically detectable in plasma for several days after a dose. Peak plasma concentrations in healthy adults reach roughly 20 nanograms per milliliter, dropping below the detection threshold of most assays within about 7 to 10 days after a single standard dose, depending on the sensitivity of the test used.
Keep in mind that “detectable” and “pharmacologically active” aren’t the same thing. The drug may be present in trace amounts in tissue for a few days longer than it shows up in blood, but at concentrations too low to have any meaningful effect.
