The reason Malarone gets away with just 7 days is precisely because it targets parasites at that early liver stage, rather than waiting for them to emerge into the blood. Even so, a delayed case of malaria is not impossible after stopping the drug. A fever developing weeks after travel to a malaria-endemic area warrants testing regardless of whether you completed your full course.
Factors That Affect How Quickly You Clear It
Several things influence how long Malarone lingers in your body. Age is one: older adults eliminate cycloguanil nearly twice as slowly as younger adults, and children clear atovaquone faster than adults do. Your genetic makeup matters too. Proguanil is broken down by a specific liver enzyme, and people who are genetically slow metabolizers through that pathway will take longer to process it.
Kidney function plays a role for the proguanil side of the equation, since up to 60% of it is excreted through the kidneys. Reduced kidney function could slow that process. Atovaquone, on the other hand, barely uses the kidneys at all. It leaves almost entirely through the gut, so kidney impairment is less of a factor for that component.
