A typical COVID infection lasts about one to three weeks for most people, though the timeline varies depending on your vaccination status, overall health, and whether you take antiviral treatment. Symptoms usually appear 2 to 14 days after exposure, with the worst of the illness concentrated in the first week.
The First Few Days: Incubation and Early Symptoms
After you’re exposed to the virus, symptoms typically take 2 to 14 days to show up. Most people notice something within 3 to 5 days. The earliest signs are usually a sore throat, fatigue, or a runny nose, followed by cough, body aches, and sometimes fever.
You’re actually contagious before you feel sick. Viral shedding begins 1 to 2 days before symptoms start, and the highest levels of virus in your body occur in the first few days of illness. This is when you’re most likely to spread it to others.
How Long Acute Symptoms Last
For most healthy adults, the acute phase of COVID (fever, cough, congestion, fatigue, body aches) runs its course within one to two weeks. Fever specifically tends to resolve within about 5 days. Most people feel noticeably better within a few weeks, though a lingering cough or fatigue can stretch beyond that.
Vaccination makes a measurable difference. In a study of healthcare workers, vaccinated individuals returned to work a median of 2 days sooner than unvaccinated individuals. They were also less likely to need more than 10 days off. At the 6-week mark, 61% of vaccinated people still reported at least one lingering symptom, compared to 79% of unvaccinated people. That’s a meaningful gap, even if it still sounds high. Most of those residual symptoms were mild, things like occasional fatigue or a scratchy throat rather than anything debilitating.
Antiviral treatment can shorten recovery further. In one large trial, people taking antivirals recovered in a median of 6 days versus 9 days with no treatment. A separate trial found a bigger spread: 14 days to full recovery with treatment compared to 21 days without. The difference likely reflects how “full recovery” was defined in each study, but the pattern is consistent. Antivirals trim days to weeks off the illness when started early.
When You Can Leave Isolation
Current guidelines no longer require a fixed 5- or 10-day isolation period. Instead, the rule is simpler: stay home until you’ve had no fever for at least 24 hours without using fever-reducing medication and your other symptoms are clearly improving. Once you meet both criteria, you can return to work, school, and daily life.
For the next 5 days after returning to normal activities, take extra precautions like wearing a mask in crowded indoor spaces and keeping distance from people who are especially vulnerable. If your fever returns or symptoms worsen after you’ve resumed activities, go back to staying home until you meet the same 24-hour fever-free threshold again, then restart that 5-day precaution window.
How Long You’re Contagious
Most people are infectious for about 8 to 10 days after symptoms begin, with the peak risk concentrated in the first few days. By the time you’ve been fever-free for 24 hours and are feeling better, your contagiousness has dropped significantly, though it hasn’t disappeared entirely, which is why the 5 days of extra precautions matter.
A positive rapid antigen test generally correlates with active contagiousness better than a PCR test does. PCR tests can remain positive for up to 90 days after infection, long after you’ve stopped being infectious. If you’re using home tests to gauge whether you’re still contagious, rapid antigen tests are the more useful tool.
COVID Rebound
Some people feel better for a few days, then develop a second wave of symptoms. This is called rebound, and it typically happens 3 to 7 days after the initial illness resolves. It occurs both in people who took antivirals and those who didn’t.
In CDC data, the median time from diagnosis to viral rebound was 9 days, and the rebound resolved around day 16 to 19 of the overall illness. Rebound symptoms are generally milder than the first round. If it happens to you, follow the same isolation guidance: stay home until you’re fever-free for 24 hours and improving, then take precautions for another 5 days.
When Symptoms Last Months: Long COVID
A significant minority of people develop symptoms that persist well beyond the acute infection. Across a pooled analysis of 159,000 participants from 19 countries, about 23% of people infected during the Omicron period reported long COVID, defined as symptoms lasting 12 weeks or more. That’s lower than the 36% seen with earlier variants, but still substantial.
The most common long-lasting symptoms are fatigue, brain fog, shortness of breath, and sleep problems. In nearly 30% of long COVID cases, symptoms persisted beyond six months. Long COVID is less likely if you’re vaccinated, younger, and had a mild initial infection, but it can happen to anyone regardless of how the acute illness felt.
There’s no single test for long COVID. It’s diagnosed based on symptoms that stick around or develop within three months of infection and can’t be explained by another condition. If fatigue, cognitive difficulty, or breathing issues are still affecting your daily life weeks after your positive test, that’s worth bringing up with a doctor rather than waiting it out.
