Rebound hypertension typically lasts 3 to 4 days, though the exact timeline depends on which medication was stopped and how abruptly. Blood pressure usually spikes within hours to a couple of days after the last dose, peaks around day 1 or 2, and gradually settles over the following days. In some cases, particularly with beta-blockers, the rebound effects can stretch out over one to two weeks.
Timeline by Medication Type
The class of blood pressure drug you were taking has a major influence on when rebound hypertension starts, how intense it gets, and how quickly it resolves.
Central-acting agents (clonidine, guanfacine, tizanidine): These medications carry the highest rebound risk. After stopping clonidine, blood pressure, heart rate, and stress hormone levels rise significantly within 12 to 24 hours, peak within 1 to 2 days, and decline after 3 to 4 days from the last dose. Guanfacine follows a slightly slower pattern, with rebound symptoms appearing 2 to 4 days after stopping. Tizanidine, which works similarly to clonidine, can trigger a spike within just hours of the last dose.
Beta-blockers (propranolol, metoprolol): The withdrawal window is longer. After stopping propranolol, blood pressure overshoot has been documented from roughly day 4 through day 14. Metoprolol tends to cause more of a heart rate overshoot than a blood pressure spike, with effects lasting from about day 2 through day 8. Notably, not all beta-blockers carry the same risk. Pindolol, for example, showed no significant rebound in comparative studies.
What Causes the Spike
When you take a medication that suppresses your sympathetic nervous system (the “fight or flight” system), your body adapts to that suppression over time. It essentially turns up the volume on its own stress signals to compensate. When the drug is suddenly removed, all that pent-up signaling floods back at once, pushing blood pressure above where it was before you started the medication. This is why rebound readings can temporarily exceed your original, untreated blood pressure levels.
With clonidine and similar drugs, this surge involves a sharp rise in norepinephrine, one of the body’s primary blood-pressure-raising chemicals. The effect is pronounced because these drugs work directly on the brain’s control center for blood pressure regulation, so withdrawal leaves the system temporarily unchecked.
Symptoms Beyond the Numbers
Rebound hypertension doesn’t always announce itself with obvious symptoms, which is part of what makes it risky. Many people feel nothing at all, and the only clue is an elevated reading on a blood pressure cuff. When symptoms do appear, they can include anxiety, headache, nosebleeds, shortness of breath, and a noticeably faster heartbeat.
More severe rebounds can push blood pressure into dangerous territory, generally above 180/110 mm Hg. At these levels, the risk of organ damage becomes real. Warning signs of a hypertensive emergency include chest pain, sudden vision changes, severe headache, confusion, dizziness, and stroke-like symptoms such as facial drooping, slurred speech, or sudden weakness on one side of the body. This level of rebound requires immediate medical attention.
How Rebound Hypertension Is Treated
The most effective treatment is usually restarting the medication that was stopped. In clinical practice, the approach is to start at a low dose and increase gradually rather than jumping back to the previous full dose. If multiple blood pressure medications were stopped at the same time, the drug with the shortest duration of action is typically restarted first, since it’s the one most likely driving the rebound.
In hospital settings, if blood pressure climbs dangerously high during a rebound episode, clinicians use fast-acting blood pressure medications as “escape” therapy to bring readings back to a safer range, generally below 180/110 mm Hg. The goal isn’t to normalize blood pressure instantly, which can be equally dangerous, but to reduce it gradually over hours.
How Tapering Prevents It
Rebound hypertension is almost entirely preventable with a proper taper. Rather than stopping abruptly, the dose is reduced in steps over a period of days to weeks, giving your nervous system time to readjust. For beta-blockers and central-acting drugs, the standard approach in clinical protocols is a tapering period of about two weeks. This is long enough to prevent the withdrawal syndrome that causes the spike.
The risk is highest when medication is stopped cold turkey, whether intentionally, by accident (running out of a prescription, for instance), or because of a misunderstanding about instructions. If you’re planning to stop or switch a blood pressure medication, the transition should always be gradual and supervised. Even medications that aren’t primarily prescribed for blood pressure, like tizanidine for muscle spasms, can cause rebound hypertension if they happen to affect the same nervous system pathways.
Elderly patients appear to be at particular risk. In one study of older adults discontinuing clonidine patches, three out of four patients experienced a rapid rise in blood pressure above their pre-treatment baseline, suggesting that age-related changes in blood vessel flexibility may amplify the rebound effect.
