Reglan (metoclopramide) is mostly cleared from your body within 25 to 35 hours after your last dose. The drug has an average elimination half-life of 5 to 7 hours, meaning half the drug is gone every 5 to 7 hours. After about five half-lives, the amount remaining is negligible. However, some side effects can linger well after the drug itself has left your bloodstream.

How Quickly Reglan Clears Your Body

When you take Reglan by mouth, it reaches peak levels in your blood within 1 to 2 hours. From there, your body starts breaking it down and filtering it out. The FDA reports an average elimination half-life of 5 to 6 hours for the injectable form and roughly 7 hours for the oral tablet. Factors That Slow Elimination

Your kidneys do most of the heavy lifting when it comes to clearing Reglan. If your kidney function is reduced, the drug stays in your system longer because your body simply can’t filter it out at the normal rate. The FDA doesn’t provide a specific extended half-life number for kidney impairment, but the effect is significant enough that doctors typically reduce the dose.

Liver disease also makes a meaningful difference. In patients with cirrhosis, plasma clearance drops by about 50%, according to the European Medicines Agency. Age plays a role as well. Very young infants process Reglan much more slowly. Side Effects Can Outlast the Drug

The more serious concern is tardive dyskinesia, a condition involving involuntary repetitive movements, often of the face, tongue, or jaw. The FDA warns that tardive dyskinesia is potentially irreversible. This risk is the primary reason the FDA caps the recommended treatment duration at 12 weeks.

The 12-Week Treatment Limit

Regardless of the condition being treated, whether it’s gastroparesis related to diabetes or gastroesophageal reflux, the FDA sets a maximum recommended treatment duration of 12 weeks. Reglan has not been shown to be safe or effective beyond that window. The longer you take it, the higher your risk of developing tardive dyskinesia, and that risk doesn’t reset when the drug leaves your bloodstream.

If your doctor determines that treatment beyond 12 weeks is unavoidable, routine monitoring for early signs of tardive dyskinesia becomes important, because catching it early and stopping the medication gives you the best chance of the symptoms resolving.
